Acht signalen maken het buikgevoel van verpleegkundigen concreet

Een zieke baby ziet er niet altijd ziek uit. Toch voelt een ervaren verpleegkundige soms dat er iets niet klopt, nog vóór de monitor alarm slaat. Om kinderen op te volgen meten verpleegkundigen onder meer hartslag, ademhaling en zuurstofgehalte. Die gegevens worden verwerkt in een waarschuwingsscore die aangeeft wanneer extra aandacht nodig is. Om dat gevoel van ‘ik vertrouw het toch niet’ concreter te maken, ontwikkelden wij een nieuwe schaal, de NIPDS-PED. Acht concrete signalen helpen om dat gevoel te onderbouwen en bleken in onze studie ongeveer even goed aan te geven welke baby’s binnen 24 uur achteruitgingen als de waarschuwingsscore die ziekenhuizen nu al gebruiken.

Op kinderafdelingen gebruiken ziekenhuizen de Pediatric Early Warning Score (PEWS), die aan de hand van meetbare signalen zoals hartslag, ademhaling en zuurstofgehalte waarschuwt voor mogelijke achteruitgang. Toch merken kinderverpleegkundigen vanuit hun ervaring soms al dat er iets niet klopt terwijl die waarden nog geen duidelijke afwijkingen tonen. Dat gevoel is niet altijd eenvoudig onder woorden te brengen, maar kan belangrijke subtiele veranderingen bij een kind weerspiegelen. Vooral bij baby’s is dat relevant, omdat zij een verslechtering een tijdlang kunnen compenseren, waarna hun toestand plots snel kan achteruitgaan.

Acht signalen om een gevoel concreet te maken

Om het gevoel van de verpleegkundigen concreter te maken, ontwikkelden we in onze masterproef een schaal met acht signalen, de Nurse Intuition Patient Deterioration Scale-Pediatrics, kortweg NIPDS-PED. We vertrokken van de wetenschappelijke literatuur en legden de mogelijke signalen voor aan twee kinderartsen en vier ervaren kinderverpleegkundigen. Samen kwamen we tot acht signalen.

Het gaat om acht vragen. Is de ademhaling veranderd? Kreunt de baby? Ziet de huid er anders uit? Zijn er tekenen van uitdroging? Is de baby opvallend suf? Is de baby erg prikkelbaar, bijvoorbeeld bij de verzorging? Kijkt het paniekerig of staart het voor zich uit? Heeft het vlekjes of puntjes die je niet kan wegdrukken?

Zo kan een verpleegkundige een ‘slecht gevoel’ bij de toestand van een kind concreet benoemen en doorgeven aan collega’s of de arts.

Een test bij 95 baby’s

Vervolgens testten we de NIPDS-PED op de kinderafdelingen van twee Antwerpse ziekenhuizen. Verpleegkundigen vulden ze in totaal 131 keer in, bij 95 baby’s jonger dan één jaar. Dat gebeurde bij opname of wanneer ze zich zorgen maakten. Tegelijk noteerden ze ook de PEWS-score.

Daarna keken we wat er de volgende 24 uur met het kind gebeurde. Had hij extra zuurstof nodig? Moest een arts worden gecontacteerd? Was extra toezicht of onderzoek nodig? Moest het kind naar intensieve zorgen?

In 60 van de 131 gevallen ging het binnen die 24 uur slechter met het kind. Verpleegkundigen vulden 33 keer de NIPDS-PED in omdat ze zich zorgen maakten. Bij 28 daarvan (85%) ging het kind binnen 24 uur klinisch achteruit. Meestal was extra zuurstof nodig; in vijf gevallen volgde een overplaatsing naar intensieve zorgen. Daarbij zagen we een duidelijk patroon. Hoe hoger een kind scoorde op onze schaal, hoe vaker het nadien slechter met hem ging.

Minstens even relevant in ons onderzoek was de vaststelling dat de nieuwe schaal het ongeveer even goed deed als de bestaande PEWS.

Het gevoel van ouders

We onderzochten ook de bezorgdheid van de ouders. Waarom? Zij kennen hun kind doorgaans beter dan wie ook. Wat bleek? Hoe groter de bezorgdheid van de ouders, hoe hoger hun kind gemiddeld ook scoorde op de NIPDS-PED.

Toch is er een belangrijke nuance. De bezorgdheid van de ouders op zich voorspelde in onze studie minder goed of hun baby achteruit zou gaan. Daarmee willen we de rol van de ouders niet negeren. Maar hun gevoelens zijn complex.  Een ouder kan bijvoorbeeld angst hebben omdat de baby voor het eerst in het ziekenhuis ligt. Die angst is niet direct een signaal van verslechtering. Het gevoel van de ouders verdient alleszins verder onderzoek.

Is de schaal gebruiksvriendelijk?

We vroegen de verpleegkundigen ook hoe gebruiksvriendelijk ze de NIPDS-PED vonden. Doorgaans gaven ze een 8 op 10. Verpleegkundigen met minder dan vijf jaar ervaring vonden de schaal gebruiksvriendelijker (9 op 10) dan meer ervaren collega’s (8 op 10). Misschien herkennen ervaren verpleegkundigen de signalen toch al en zien ze minder nut in een extra lijst op papier. Of helpt de schaal jonge verpleegkundigen net om hun buikgevoel ernstig te nemen? Dat konden we in dit onderzoek niet nagaan.

Hand in hand met meetgegevens 

De NIPDS-PED is nog geen instrument dat zonder meer op elke kinderafdeling kan worden ingevoerd. Ons onderzoek was relatief klein en vond plaats in de wintermaanden in twee ziekenhuizen. Verder onderzoek bij meer kinderen en in meer ziekenhuizen is nodig.

Maar, onze studie toont wel aan dat het buikgevoel van de verpleegkundige niet mag worden onderschat, en niet vaag hoeft te blijven. Met concrete observaties kan dat gevoel beter worden benoemd en gedeeld.

Dat betekent niet dat de NIPDS-PED de PEWS moet vervangen. Meetbare resultaten vertellen wat er verandert in het lichaam. Een verpleegkundige ziet daarnaast hoe een baby zich gedraagt, eruitziet en reageert.

De vraag hoeft daarom niet te zijn: vertrouwen we op mensen of vertrouwen we op cijfers? Misschien ligt de meerwaarde net in de combinatie van beide.

Bij een baby wiens gezondheid verslechtert, kan het belangrijkste signaal soms beginnen bij iemand die simpel opmerkt: deze baby is niet meer zoals daarnet. De NIPDS-PED kan helpen om die waarneming en dat gevoel concreet te maken.
