Visualisatie en de Bevallingservaring

Visualiseer je baby op de wereld

Slechts iets meer dan de helft van de mama’s die in het ziekenhuis zijn bevallen, ervaren hun bevalling positief. Dat blijkt uit onderzoek. Heel wat vrouwen kijken blijkbaar met een dubbel gevoel terug naar de geboorte van hun kind: ze herinneren zich de stress of angst bij de beginnende weeën, de pijn bij het persen of de teleurstelling omdat de omstandigheden niet helemaal naar verwachting waren.

Nochtans bestaan er verschillende natuurlijke ontspannings- en pijnstillingstechnieken die een positieve invloed hebben op de bevallingservaring. Sommige van die technieken hebben tot nu toe nog weinig wetenschappelijke omkadering gekregen: zo ook visualisatie.

Van positieve gedachten naar een positief resultaat

In de sport- en zakenwereld is het concept visualisatie al langer bekend. Bij visualisatie wordt een situatie van het begin tot het einde ingebeeld. Het eindresultaat is daarbij positief: er wordt toegewerkt naar het gewenste doel. Een sporter die traint voor zijn eerste marathon visualiseert zichzelf bijvoorbeeld aan het begin van het parcours. Hij ziet het publiek links en rechts van hem. Hij beeldt zich in hoe hij voorbij de verschillende mijlpalen loopt: 10 km, 20 km, 30 km en 40 km. Dan ziet hij zichzelf 200 m voor de finish en kijkt hij naar de klok waar zijn doeltijd op staat. Vervolgens loopt hij succesvol over de finishlijn. De sporter herhaalt dit beeld totdat zijn onderbewustzijn gelooft dat het de werkelijkheid is. Via het vertrouwen dat hieruit vloeit stijgt de kans dat het gewenst resultaat wordt bereikt.

Ook tijdens de bevalling kan de geest het lichaam op deze manier ondersteunen. Neem bijvoorbeeld de visualisatieoefening van ‘de bloeiende bloem’. De baarmoederhals die opent wordt hierbij voorgesteld als een bloem die zich opent, bloemblaadje per bloemblaadje, tot ze volledig open is. Bij dat beeld kunnen nog verschillende details toegevoegd worden: de vorm van de bloemblaadjes, dauwdruppels op de bloem, de geur van de bloem, vogelgeluiden in de verte of een krachtige zonnestraal op de bloem.

De zwangere vrouw beleeft deze visualisatie door bijvoorbeeld herhaaldelijk, voor en tijdens haar bevalling, een audiofragment te beluisteren dat haar door deze beeldvorming begeleidt. Het beeld van de bloem straalt warmte, schoonheid, zachtheid en opening uit en stelt de vrouw op haar gemak: de bevalling is een prachtig, natuurlijk proces waar geen angst, stress of pijn aan te pas moet komen. Haar lichaam zal zich hierdoor automatisch klaarmaken voor een vlotte bevalling.

Van vluchtreactie naar vertrouwen

De impact van de geest op het lichaam kan grotendeels verklaard worden vanuit het zenuwstelsel en de hormonenspiegel. Een uitgewerkt voorbeeld hiervan is de angst-spanning-pijncyclus van dr. Grantly Dick-Read. Een vrouw die angstig of gestrest is voor haar naderende bevalling, bijvoorbeeld door horrorverhalen van haar vriendinnen, zal haar weeën als een bedreiging ervaren. Als reactie hierop zet het zenuwstelsel de aanmaak van het stresshormoon adrenaline in gang. Dit hormoon zorgt voor de versnelling van de hartslag en ademhaling en het aanspannen van de spieren. Deze reactie stamt uit de oertijd waarin een vrouw in het bos beviel en in staat moest zijn om snel te vluchten in het geval van bedreiging, bijvoorbeeld door een naderende beer.

Ook de baarmoederspieren spannen zich op, en doordat de weeën en de baby daar druk op uitoefenen, resulteert dat in pijn. Druk die uitgeoefend wordt op spieren in spanning doet immers meer pijn dan druk op spieren in ontspanning. Bovendien worden, onder invloed van de adrenaline, vooral de beenspieren van bloed voorzien, zodat de vrouw zou kunnen weglopen van de bedreiging. Hierdoor wordt er minder bloed naar de baarmoeder gepompt. En een spier die onvoldoende zuurstof krijgt, veroorzaakt ook pijnlijke krampen.

Angst zorgt dus voor spanning en die spanning zorgt voor meer pijn. Die verhoogde pijn zorgt dan weer voor meer angst, omdat de zwangere het gevoel heeft dat ze de weeën onvoldoende kan opvangen en schrik heeft voor wat er nog gaat komen. En zo begint de cyclus weer opnieuw.

Bij visualisatie worden negatieve gedachten vervangen door positieve gedachten, die vervolgens worden omgezet in beelden. Angsten en negatieve blokkades worden losgelaten en vrouwen krijgen meer vertrouwen in hun lichaam en in het geboorteproces. Door dat verhoogde vertrouwen kunnen ze zich meer ontspannen.

Indien de vrouw zich kan ontspannen tijdens de arbeid komen er als reactie op de oxytocine die vrijkomt endorfines, gelukhormonen, vrij. Dankzij de endorfines komt de vrouw in een soort zone of roes terecht, waar ze niet wordt afgeleid door prikkels van buitenaf. In die zone kan ze het bevallingsproces accepteren zoals het is: ze geeft er zich aan over. In die zone wordt de pijn van de weeën ook verlicht: endorfines zijn immers lichaamseigen pijnstillers. Zo ontstaat er in plaats van een angst-spanning-pijncyclus een cyclus van vertrouwen-ontspannen-verlichten.

Voordelen van visualisatie voor de vroedvrouw

De onderzoeken rond visualisatie zijn voorlopig beperkt en kleinschalig, maar de resultaten zijn veelbelovend. Ze bevestigen onder meer dat visualisatie stress en angst vermindert, de bloeddruk verlaagt en ontspannend werkt. Soortgelijke resultaten zijn ook terug te vinden bij onderzoek rond mindfulness en hypnose, ontspanningstechnieken die een nauwe band hebben met visualisatie en al langere tijd met succes worden toegepast als natuurlijke pijnstilling bij bevallingen.

Visualisatie is bovendien een krachtige tool die de vroedvrouw voor en tijdens de bevalling kan inzetten om een veilige en geborgen omgeving te creëren waarin de vrouw zich vol zelfvertrouwen kan ontspannen. Ook helpt visualisatie de vrouw om op voorhand na te denken over haar geboortewensen. Zo kan ze goed communiceren met de zorgverleners over haar noden en wensen. Natuurlijk moet ze daarin realistisch en flexibel blijven: soms gaan zaken niet volledig als gewenst. Ook hierbij speelt visualisatie een positieve rol: dankzij de ontspanningsroes die het in gang zet, accepteert de vrouw makkelijker onverwachte wendingen. Belangrijk is dat de vrouw zich gesteund en empowered voelt door de zorgverleners en betrokken mag zijn in beslissingsprocessen. Kortom: visualisatie heeft een duidelijk positief effect op de bevallingservaring. Het is voor de vroedvrouw dan ook een meerwaarde om deze techniek verder te verkennen en toe te passen in de praktijk.
