Het verband tussen voedingsgewoonten bij patiënten met een bariatrische chirurgie en de evolutie van NAFLD: Pre- en postoperatieve vergelijking

Vettig eten, vette lever?

Elk jaar sterven wereldwijd 2,8 miljoen mensen aan de complicaties van overgewicht en obesitas. In België lijdt 29% van de bevolking aan overgewicht en is 16% zelfs obesitas. De classificatie ondergewicht, overgewicht en obesitas wordt gedaan aan de hand van de body mass index (BMI). De BMI bepaalt de verhouding tussen de lengte en het gewicht. Deze wordt berekend door het gewicht in kilogram te delen door het kwadraat van de lengte in meter. Obesitas klasse I kan worden vastgesteld vanaf een BMI van 30-34,9 kg/m2. Obesitas klasse II wordt gediagnosticeerd bij een BMI van 35-39,9 kg/m2. Vanaf een BMI van ≥40 kg/m2 is er sprake van morbide obesitas.

Eén van de veelvoorkomende complicaties bij obesitas is een vervette lever, ook wel non alcoholic fatty liver disease (NAFLD) genoemd. De vraag in dit onderzoek is: Welke rol speelt voeding in de evolutie van een vervette lever?

Stadia leververvetting

NAFLD wordt gedefinieerd als een leveraandoening waarbij minimaal 5% van de lever vervet is. Bij het stellen van de diagnose is het belangrijk om uit te sluiten dat een verhoogde alcoholinname de oorzaak is de leververvetting. 

Wanneer  er sprake is van een vervette lever in combinatie met ontstekingen spreken we van on alcoholic steatohepatitis (NASH). NASH kan op zijn beurt verder evalueren naar fibrosis, dit is een vervette lever met zowel ontstekingen als littekenweefsel. Het grote verschil tussen deze stadia is dat een vervette lever zonder ontstekingen makkelijker te behandelen is dan een vervette lever met ontstekingen. 

 Evolutie NAFLD ( (Dr. Moore, 2010)

Oorzaak

De oorzaak van leververvetting is volgens de wetenschap nog niet helemaal duidelijk. Een sedentaire levensstijl en ongezond voedingspatroon is hoogst waarschijnlijk de grootste oorzaak. Omdat  er bij een vervette lever geen sprake is van specifieke symptomen, kunnen artsen niet gemakkelijk een diagnose stellen. Soms kunnen er vage klachten optreden zoals misselijkheid en vermoeidheid, maar meestal is deze aandoening niet gelinkt aan specifieke symptomen.

Minder kilo’s, minder vet?

De behandeling van een vervette lever bestaat hoofdzakelijk uit een gezond en evenwichtig voedingspatroon in combinatie met voldoende fysieke activiteit. Gewichtsverlies zou een in verband staan met de daling van het aantal vervette cellen in de lever. 

Binnen de morbide obese populatie (BMI > 40 kg/m2) zou bariatrische chirurgie de enige effectieve methode zijn om gewicht te verliezen op lange termijn. Bariatrische chirurgie is een tak binnen de geneeskunde die zich bezighoudt met het behandelen van zwaarlijvigheid. Er bestaan verschillende soorten bariatrische chirurgie. De meest voorkomende ingrepen zijn; Laparoscopic Sleeve gastrectomie (LSG) en de Roux- en Y gastric bypass (RYGB).

Bij een Laparoscopic Sleeve gastrectomie wordt een groot deel van de maag verwijderd en blijft er een maagbuis over. Deze maagbuis heeft een inhoud van ongeveer 150 ml. Bij een de Roux- en Y gastric bypass wordt met behulp van nietjes het bovenste gedeelte van de maag een klein maagreservoir gecreëerd. Dit ‘nieuwe’ reservoir heeft geen doorgang meer met de rest van de maag. Via een smalle doorgang gaat het voedsel van de ‘nieuwe’ maag rechtstreeks naar de dunne darm. Hierdoor wordt het voedsel minder goed verteerd en blijft het niet lang in de maag zitten.

Sleeve Gastrectomy en Roux-en-Y Gastric Bypass (Pories, sd)

Verschillende wetenschappers concludeerden dat gewichtsverlies na een bariatrische chirurgie een positieve invloed zou hebben op een vervette lever. Er werd aangetoond dat het aantal vervette cellen in de lever na een maagverkleining daalt.

Impact van ongezonde voeding

Een voedingspatroon rijk aan verzadigde vetten zou in verband staan met een verhoogde kans op het ontwikkelen van een vervette lever. Verzadigde vetten worden ook wel de 'slechte' vetten genoemd. Ze hebben een negatieve invloed op het cholesterolgehalte, het hart en de bloedvaten.

Verder zou de kans op leververvetting ook stijgen bij een voedingspatroon rijk aan geraffineerde koolhydraten en fructose. Koolhydraten zijn suikers die voorkomen in bijvoorbeeld brood, aardappelen, pasta en rijst. Koolhydraten worden hoofdzakelijk opgedeeld in enkelvoudige en meervoudige suikers. Fructose behoort tot de enkelvoudige koolhydraten en komt voor in verschillende voedingsmiddelen zoals siropen, tafelsuikers, honing, andere bewerkte voedingsmiddelen en fruit. Geraffineerde koolhydraten worden beschreven als suikers die een industrieel zuiveringsproces hebben ondergaan. Deze koolhydraten worden in verschillende studies geassocieerd met gezondheidsproblemen zoals leververvetting.

Het onderzoek

Deze studie werd uitgevoerd in het AZ Sint-Maarten te Mechelen. Patiënten die een Laparoscopic Sleeve gastrectomie (LSG) of een Roux- en Y gastric bypass (RYGB) ondergingen werden geselecteerd en gescreend op een vervette lever aan de hand van de ‘Hepatic Steatosis Index’. Dit is een tool die aan de hand van verschillende parameters kan berekenen of er sprake is van leververvetting. De graad van leververvetting wordt uitgedrukt in een bepaalde score. In totaal werden 12 patiënten geïncludeerd in het onderzoek.

De voedingsgewoonten van de patiënten werden bevraagd  aan de hand van een gestandaardiseerde voedselfrequentievragenlijst. Zowel de voedselfrequentievragenlijst als de ‘Hepatic Steatosis Index’ werden vóór en drie maanden na de ingreep uitgevoerd en vergeleken met elkaar.

Het doel van dit onderzoek was om de pre- en postoperatieve voedingsgewoonten in verband te brengen met de evolutie van leververvetting.

Beter eten, goed genezen?

Uit de resultaten van dit onderzoek blijkt dat het aantal vervette levercellen drie maanden na de ingreep is gedaald. Slechts bij drie voedingsmiddelen is er een verband waargenomen tussen de consumptie ervan en het aantal vervette levercellen.

Er werd een verband  aangetoond tussen de verhoogde preoperatieve consumptie van bewerkt vlees en het aantal vervette levercellen. Ook postoperatief is er een verband waargenomen tussen de verminderde consumptie van boter, de verhoogde consumptie van fruit en de evolutie van het aantal vervette levercellen.

Verder onderzoek

Omdat er in dit onderzoek slechts een beperkt aantal patiënten werden geïncludeerd en slechts drie maanden na de operatie werden opgevolgd, kon men maar een beperkt aantal verbanden aantonen. Daarom is verder onderzoek noodzakelijk om te bepalen of er wel degelijk een verband is tussen de pre- en postoperatieve voedingsgewoonten en de evolutie van leververvetting. In toekomstige studies wordt best gewerkt met een uitgebreidere populatie en worden er best ook meer opvolgletingen geïncludeerd.
