Het CRISPLD2-gen en zijn rol in aangezichtsspleten, een overzicht

Inleiding: Orofaciale schisis, wat is dit?

Orofaciale schisis is een wereldwijd veelvoorkomende aandoening waarbij baby’s geboren worden met spleten in het aangezicht, meer specifiek in hun lip, kaak en/of gehemelte.

Dit heeft een negatieve invloed op de ontwikkeling van deze kinderen. Onder andere kunnen baby’s moeilijker eten en oorproblemen ontwikkelen met als gevolg gehoorverlies en leerachter­stand. Bovendien worden ze vaak gepest omwille van hun uiterlijk. Vandaag de dag worden baby’s met aangezichtsspleten tot en met hun twintigste jaar opgevolgd door een multidisciplinair artsenteam. Hoe uit­gebreider de kennis van deze aandoening, hoe meer de behandelingen kunnen verbeterd worden. We weten al dat zowel een fout in het DNA, de drager van erfelijke informatie van een individu, als omgevingsfactoren een rol kunnen spelen in het ontstaan van aangezichtsspleten. Ze zouden ervoor zorgen dat de fusie die noodzakelijk is voor het vormen van het aangezicht in embryo’s faalt.

Het CRISPLD2-gen en onderzoek ernaar

“Het CRISPLD2-gen en zijn rol in orofaciale schisis: een literatuurstudie” maakt een synthese van de reeds gekende literatuur over het gen CRISPLD2 en zijn mogelijke rol in aangezichtsspleten. CRISPLD2 is een stukje DNA dat informatie bevat voor de aanmaak van CRISPLD2-eiwit. Eiwitten kunnen vergeleken worden met de bouwstenen van ons lichaam. Dit gen werd bestudeerd aan de hand van verschillende technieken, zoals o.a. associatiestudies en dierexperimenteel onderzoek. Bij associatiestudies wordt het DNA van een heel aantal individuen met en zonder aandoening vergeleken. Hierdoor kunnen verschillen/mutaties opgespoord worden en aan een bepaalde aandoening gelinkt worden. In dierexperimenteel onder­zoek worden laboratoriumdieren gekweekt met bepaalde mutaties om de effecten ervan te onderzoeken.

Functies van het CRISPLD2-eiwit

Studies toonden aan dat crispld2 een rol zou spelen bij de ontwikkeling van o.a. de longen, de nieren en het aangezicht. Het zou ervoor zorgen dat cellen zich gaan vermenig­vuldigen en transformeren, wat groei mogelijk maakt. Crispld2-eiwit zou ook een rol spelen in het immuunsysteem dankzij zijn LCCL domein, een klein onderdeel van het eiwit. Het zou de immunologische reactie van het lichaam uitgelokt door lipopoly­sachariden, een component van Gram-negatieve bacteriën, blokkeren. Hierdoor zou de kans op een septische shock verminderen. Over de exacte functie van het LCCL domein is tot op heden weinig geweten. Op basis van onderzoek op andere eiwitten met hetzelfde LCCL domein kan gesteld worden dat naast een functie in het immuunsysteem, het ook een rol zou kunnen spelen in celadhesie. Dit laatste is een mogelijke denkpiste in de vraag naar de rol van het CRSPLD2-gen in aangezichtsspleten. Belangrijk is te beseffen dat bovenstaande kennis gebaseerd is op laboratoriumex­perimenten op dieren of cellen in cultuur. Er bestaan hierover tot op heden geen studies op mensen. Deze hypotheses mogen dus nog niet als bewezen beschouwd worden voor de mens. Zo toonden bijvoorbeeld een aantal studies aan dat het crispld2-eiwit geproduceerd wordt in het hoofdgebied bij muis- en zebravisembryo’s. Er mag dus verondersteld worden dat dit ook zo zou kunnen zijn voor de mens, maar dit zal pas met zekerheid geweten zijn wanneer verschillende studies dit onderzocht en bevestigd hebben.

CRISPLD2 en zijn rol in aangezichts­spleten

De exacte rol van het CRISPLD2-gen in aangezichtsspleten is tot op heden niet gekend. Mutaties zouden de concentratie aan eiwit kunnen beïnvloeden, wat een negatief effect zou kunnen veroorzaken op de aangezichtsontwikkeling. Een dierexperi­menteel onderzoek uit 2012 toonde aan dat zebravisembryo’s, die genetisch gemani­puleerd waren opdat er geen crispld2-eiwit geproduceerd kon worden, misvormingen vertoonden van hun onderkaak. Ook hebben een aantal studies de link van het CRISPLD2-gen met aangezichtsspleten in verschillende populaties aan de hand van associatiestudies onderzocht. Een link werd gevonden in de Noord-Chinese, Noordwest Chinese, Turkse, Kaukasische, Braziliaanse en de Latijns-Amerikaanse populatie. Het feit dat er niet in elke populatie een link gevonden wordt, kan verklaard worden door de verschillende genetische achtergrond.

Besluit

Aan de hand van de huidige literatuur kan de hypothese gesteld worden dat het CRISPLD2-gen een rol kan spelen in het ontstaan van aangezichtsspleten. Onder andere de link in verschillende populaties, de mogelijke rol in vermenigvuldiging en transformatie van cellen in de ontwikkeling van het aangezicht en de aanwezigheid van een LCCL domein ondersteunen deze hypothese. Er is echter meer onderzoek nodig om de huidige kennis over het CRISPLD2-gen te verbreden en de hypotheses na te gaan. In de verdere toekomst, als zou blijken dat CRISPLD2 een belangrijke rol speelt in aangezichtsspleten, kan dit tot nieuwe behandelingen leiden voor deze aandoening.
