Aan het werk met een psychische kwetsbaarheid. Wat werkt?

Iedereen kijkt na een uitputtende werkdag wel eens uit naar zijn vrije dag. Voor mensen zonder werk ligt dit anders, zij kijken net uit naar die dag dat ze kunnen gaan werken. Een betaalde job vervult immers enkele noodzakelijke functies in het leven. Naast financiële zekerheid zorgt het voor een vaste structuur, sociale contacten, zingeving en een toename van het zelfvertrouwen. De laatste jaren groeit het bewustzijn dat dit niet anders is voor mensen met een psychische kwetsbaarheid. Een job hebben kan psychische klachten doen afnemen, terwijl werkloosheid ze net versterkt. Dit onderzoek focust op het Individuele Plaatsing & Steun (IPS) model. Een wetenschappelijk ondersteunde methodiek die gelooft dat iedereen betaald kan werken indien men de juiste functie en werkomgeving vindt.

Enkele cijfers

Uit onderzoek van het Wetenschappelijk Instituut Volksgezondheid (2013) blijkt dat één op de drie Belgen in zijn of haar leven getroffen wordt door een lichte tot ernstige psychische problematiek. Dit betekent zo’n 700.000 mensen per jaar. Psychische problemen brengen bovendien een kostenplaatje met zich mee, zowel voor het individu als voor de maatschappij. In België ligt de maatschappelijke kostprijs van psychische stoornissen rond de  4% van het BBP. De grootste kosten liggen echter niet in de zorgsector, maar in de werkloosheid en het ziekteverzuim. Momenteel ligt de werkloosheidsgraad van mensen met psychische kwetsbaarheid nog steeds significant hoger dan bij de rest van de bevolking, respectievelijk 18% tegenover 7%.

De IPS-basisprincipes als vertrekpunt

Het IPS-model is een wetenschappelijk onderbouwde methodiek die bewezen heeft dat het de tewerkstellingskansen van mensen met een psychische kwetsbaarheid vergroot. Dit model is ontstaan in de jaren ’90 in Amerika. Ondertussen is het wijdverspreid en wordt het sinds februari 2018 ook geïmplementeerd in België als een vijfjarig pilootproject. Binnen het IPS-model staan acht basisprincipes centraal die zo nauwgezet mogelijk moeten nageleefd worden om tot goede resultaten te komen:

  • De focus ligt op het zoeken van een reguliere betaalde job.
  • Zero-exclusion: iedereen kan een traject opstarten als hij/zij dit wil.
  • Er is een geïntegreerde samenwerking tussen de zorgsector en de IPS-coach.
  • De wens van de klant staat centraal.
  • Er wordt ondersteuning geboden over de gevolgen van betaald werk op het inkomen.
  • Het place-then-train principe wordt gevolgd. Dit betekent meteen een job zoeken op de reguliere arbeidsmarkt zonder langdurige training vooraf.
  • De IPS-coach zet in op het uitbreiden van het werkgeversnetwerk.
  • De ondersteuning is van onbepaalde duur.

In dit onderzoek wordt dieper ingegaan op twee van de acht basisprincipes: (1) Een geïntegreerde samenwerking tussen de professionele arbeidsre-integratiediensten en de zorgsector. Het IPS-model impliceert immers dat beide diensten intensief moeten samenwerken en op regelmatige basis formeel en informeel moeten afstemmen. (2) Het place-then-train principe. IPS streeft ernaar om meteen een job te zoeken op de reguliere arbeidsmarkt, waarbij de betrokkene wordt opgeleid op de werkvloer.

Uit voorgaand onderzoek is gebleken dat deze twee principes cruciaal zijn voor een succesvolle IPS-implementatie, maar dat beiden beperkt toegepast worden in Vlaanderen. De bedoeling van dit onderzoek is om een eerste verkennende studie uit te voeren naar de percepties van de zorgsector (psychologen, psychiaters, sociaal werkers, …) en ervaringsdeskundigen (mensen met een psychische kwetsbaarheid) omtrent deze twee IPS principes.

Resultaten & Conclusies

  1. Een gestructureerde samenwerking?

Uit dit onderzoek blijkt dat er nog geen gestructureerde samenwerking is tussen de zorgsector en de arbeidsre-integratiediensten. Ondanks verschillen tussen de bevraagde organisaties situeert de bestaande samenwerking zich vooral op individueel niveau, namelijk binnen het traject van een klant. Door de zorgverleners en ervaringsdeskundigen werd wel een engagement getoond om deze samenwerking verder te optimaliseren. Een ervaringsdeskundige pleitte voor infosessies rond de mogelijkheden van arbeidsre-integratie vanuit de psychiatrie, en een zorgverlener opperde het idee van een ‘mobiele’ arbeidscoach in hun zorgteam. Onderstaand citaat maakt duidelijk dat zijn voorstel sterke gelijkenissen vertoont met de verantwoordelijkheden van een jobcoach binnen het IPS-model.

“Een arbeidscoach die betrokken is, die ook eens op huisbezoek gaat, die een voorstel doet naar een werksituatie, die een opleiding voorstelt, die dat wat opvolgt, die een terugval mee maakt met de cliënt op zich. Ik denk dat dat een heel groot verschil zal maken voor de cliënten. Een arbeidscoach die dichter staat en niet enkel op de bureau zit.” (Zorgverlener, Focusgroep 2)

  1. Het place-then-train principe

Uit de focusgroepen blijkt enerzijds dat zorgverleners en ervaringsdeskundigen een stapsgewijs en laagdrempelig voortraject verkiezen. Dit zorgt voor minder werkdruk, minder stress en een veilige ‘probeer’ omgeving. Anderzijds heeft een traject meteen gericht op een betaalde tewerkstelling ook verschillende voordelen volgens hen, zoals een toename van het zelfvertrouwen en financiële vooruitgang. Binnen de focusgroepen was er geregeld verwarring omtrent een traject meteen naar betaald werk. Is er dan nog begeleiding mogelijk bijvoorbeeld? Een duidelijke omschrijving van het place-then-train principe is noodzakelijk. Dit principe streeft zo snel mogelijk naar een betaalde job, maar omvat ondersteuning op maat van de klant. Het is met andere woorden niet zomaar een traject naar betaald werk, zonder meer.

Het vinden en behouden van een betaalde job is voor iedereen van ons een belangrijk aspect in het leven. De implementatie van het IPS-model in België kan ertoe bijdragen dat ook mensen met een psychische kwetsbaarheid gemakkelijker hun weg naar de arbeidsmarkt (terug) vinden. Op die manier kunnen ook zij – na een lange werkdag – eens uitkijken naar die vrije dag.
