Hypospadias repair in adults: are the results different in comparison with children? Identification of prognostic factors

Problems down under: waarom sommige penissen goed opgevolgd moeten worden

De meeste mannen (en vrouwen) beschouwen de penis als een bron van plezier. Toch is dat voor sommigen niet het geval. Er bestaan namelijk een hoop aandoeningen van de penis die voor ernstige problemen kunnen zorgen en de levenskwaliteit van mannen grondig kunnen verstoren. Een vaak voorkomend voorbeeld van zo’n aandoening is hypospadie.

Hypospadie is de wetenschappelijke benaming van een aangeboren aandoening waarbij de plasbuis niet op de tip van de eikel uitmondt, maar ergens lager op de penis. Meestal ontbreekt er ook een deel van de voorhuid en is er sprake van een kromstand. Omdat de afwijkingen al vanaf de geboorte aanwezig zijn, wordt de diagnose vaak vroeg in het leven gesteld. Hoewel er in sommige milde gevallen geen behandeling noodzakelijk is, willen ouders vaak een operatie voor hun zoon om de afwijking te herstellen. De meeste hypospadiepatiënten worden daarom geopereerd tijdens de kindertijd.

Hypospadie: een aangeboren aandoening van de penis.

Moeilijke operatie met kans op complicaties

Bij zo’n operatie wordt een extra stuk plasbuis aangelegd zodat die op de tip van de eikel kan uitmonden. Tegelijkertijd wordt de kromstand verholpen. Op deze manier krijgen de meeste patiënten na een of twee ingrepen een cosmetische standaardpenis die ook normaal functioneert. Helaas kunnen patiënten na de operatie ook complicaties ontwikkelen. Het gaat dan bijvoorbeeld om een gaatje dat zich tussen de huid en de nieuwe plasbuis bevindt of om een vernauwing van die plasbuis. In beide gevallen is een extra ingreep noodzakelijk.

Bij kinderen is er al veel onderzoek gedaan naar het optreden van complicaties na een operatie voor hypospadie. Toch moeten sommige patiënten ook op volwassen leeftijd nog operaties ondergaan om complicaties van eerdere, gefaalde ingrepen te herstellen. In uitzonderlijke gevallen worden patiënten zelfs pas op volwassen leeftijd voor de eerste keer geopereerd. Voornamelijk patiënten met milde gevallen van hypospadie ondergaan in eerste instantie geen ingreep omdat de aandoening hen weinig last bezorgt. Op latere leeftijd beslissen ze dan toch om zich te laten opereren.

Meer complicaties bij volwassenen

Aangezien er weinig bekend is over volwassen hypospadiepatiënten, is er tijdens dit onderzoek bestudeerd hoeveel complicaties zich bij deze patiëntengroep voordoen. In totaal deden 65 mannen mee aan het onderzoek. De resultaten zijn zorgwekkend: slechts 36 procent van alle operaties bij deze volwassenen heeft niet geleid tot complicaties. Dat staat in schril contrast met eerder gevoerd onderzoek bij kinderen, waarbij gemiddeld 64 procent van alle operaties succesvol blijkt te zijn. Gelukkig ziet men dat hoe meer operaties een volwassen patiënt ondergaat, hoe meer kans hij heeft om later geen complicaties meer te ontwikkelen. Toch zullen uiteindelijk slechts 64 procent van de volwassenen een gunstig resultaat bekomen na hun laatste operatie. Bij kinderen bedraagt dat 84 procent.

Bovendien blijkt uit verdere analyse van deze resultaten dat complicaties zich tot lang na de operatie kunnen ontwikkelen. Gemiddeld gezien worden complicaties twee jaar na de operatie opgemerkt. Toch worden in sommige gevallen complicaties pas na meer dan tien jaar ontdekt.

Artsen trekken aan de alarmbel

De resultaten van dit onderzoek bieden belangrijke nieuwe inzichten in hoe de lange termijnopvolging van hypospadiepatiënten georganiseerd moet worden. Momenteel worden patiënten op zes weken en op zes maanden na de ingreep terug op consultatie verwacht om eventuele problemen op te sporen. Als de patiënt geopereerd werd vooraleer hij zindelijk was, worden ook op vier- en op zevenjarige leeftijd afspraken ingepland om de plasstraal te beoordelen. Tot slot komen patiënten na de puberteit, meestal rond de leeftijd van veertien à vijftien jaar, nog een laatste keer op consultatie. Eventuele problemen worden dan besproken, waarbij er ook aandacht is voor de erecties van de patiënt. Als er geen problemen zijn, worden geen vervolgafspraken meer gepland. Toch tonen de resultaten van dit onderzoek aan dat ook tijdens het volwassen leven verdere opvolging voor sommige patiënten nodig blijft.

Nu de wachttijd om een specialist te consulteren soms meerdere maanden bedraagt, valt een stelselmatige opvolging van alle hypospadiepatiënten niet te organiseren, noch te verantwoorden. Hypospadiepatiënten moeten echter wel optimaal geïnformeerd worden over hun situatie. Zo moeten ze beseffen dat er een risico bestaat op laattijdige complicaties. Daarenboven moet het voor hen mogelijk zijn om vroegtijdig doorverwezen te worden naar de uroloog. Een snelle doorverwijzing is belangrijk, want hoe langer een complicatie onbehandeld blijft, hoe moeilijker het wordt om ze te herstellen.

Ook de huisarts kan hierin een cruciale rol spelen. Hij of zij is het eerste aanspreekpunt waarop patiënten een beroep kunnen doen. Het vroegtijdig (h)erkennen van problemen die mogelijk een gevolg zijn van eerdere operaties kan zorgen voor een laagdrempelige en gerichte verwijzing naar de uroloog.

Waarom uitstellen geen goed idee is

In veel medische specialismen wordt de laatste jaren een trend gezien waarin de behandeling van sommige niet-levensbedreigende aandoeningen bij minderjarige patiënten wordt uitgesteld. Ouders moeten namelijk vaak beslissingen nemen over de behandeling van hun minderjarig, niet-wilsbekwaam kind. Door de behandeling uit te stellen totdat de patiënt wilsbekwaam is, kan hij zelf beslissen of hij de behandeling al dan niet wenst te ondergaan. Dezelfde trend heeft zich de laatste jaren ook ingezet bij de behandeling van hypospadiepatiënten. De resultaten van dit onderzoek wijzen er echter op dat een operatie voor hypospadie uitstellen tot op volwassen leeftijd geen goed idee is.

Geneeskunde is een wetenschap in voortdurende evolutie. Nieuwe ontdekkingen leiden tot een meer zorgvuldige diagnosestelling en tot betere therapieën. De nieuwe inzichten uit dit onderzoek moeten ervoor zorgen dat ook patiënten met hypospadie door een doeltreffende aanpak ten volle kunnen genieten van hun geslachtsorgaan.
