Ondanks zijn adaptieve functies, wordt angst vaak gezien als iets negatiefs. Wanneer we gevaar ervaren, zorgt angst ervoor dat ons lichaam gepast reageert door bijvoorbeeld te vechten of te vluchten. Maar wanneer wordt het dan problematisch? Wanneer mensen ook bang worden van niet-bedreigende stimuli of situaties en deze angst een ernstige impact heeft op hun dagelijkse leven. In 2015 leefden wereldwijd naar schatting 264 miljoen mensen met een angststoornis. Onderzoek toont aan dat het fenomeen waarbij angstresponsen uitgelokt worden door andere, gelijkende stimuli, ook wel angstgeneralisatie genoemd, meespeelt in het ontstaan en in stand houden van angststoornissen. Maar wat veroorzaakt nu deze generalisatie en wat maakt dat sommige mensen hier gevoeliger voor zijn dan anderen?

De oorzaken

Het probleem is dat de mechanismen die deze generalisatie veroorzaken nog niet helemaal duidelijk zijn. Sommige onderzoeken tonen aan dat angstige personen meer geneigd zijn om dubbelzinnige stimuli te interpreteren als bedreigend en als gevolg in nieuwe situaties overmatig angstig reageren om potentiële risico’s te vermijden. Recent onderzoek vindt dat conditionering (dit is het leerproces waarbij een neutrale stimulus een respons uitlokt nadat deze gepaard ging met een aversieve of appetitieve stimulus) het vermogen om de initieel vreesuitlokkende stimulus te onderscheiden van een nieuwe stimulus negatief kan beïnvloeden en dat het onvermogen om verschillende stimuli van elkaar te onderscheiden vaak groter is bij angstpatiënten. Dit onderzoek suggereert een sterke aanwijzing voor de rol van perceptie in de generalisatie van vrees maar deze rol is nog maar weinig onderzocht.

Het doel van deze studie was om de rol van perceptie verder te onderzoeken in een doelgroep van mensen zonder angstklachten. In een vreesconditioneringsparadigma werd er eerst angst geïnduceerd door een cirkel te combineren met een aversieve stimulus. Hierna werden participanten blootgesteld aan zeven cirkels met verschillende groottes (Generalization Stimulus, GS) en moesten ze aangeven of de gepresenteerde cirkel de initiële vreesinducerende stimulus (Conditioned Stimulus, CS) was of een nieuwe cirkel (GS). Ze moesten ook telkens aangeven in welke mate ze een elektrische prikkel verwachtten. De verwachting van de elektrische prikkel diende als subjectieve vreesmeting, de oogknipperreflex en de huidgeleiding als objectieve metingen. Door deze categorisatie taak toe te voegen konden vreesresponsen die volgden op cirkels die gepercipieerd werden als de CS onderscheiden worden van de cirkels die gepercipieerd werden als een nieuwe cirkel en kon het aandeel van perceptie in de generalisatie van vrees nagegaan worden.

We vonden dat nieuwe cirkels (GS) vaak fout gepercipieerd werden als de initiële vreesinducerende stimulus (CS) en dat deze misperceptie ervoor zorgde dat vrees gegeneraliseerd werd naar andere stimuli (GS). Dit vonden we zowel terug in de psychofysiologische maten als in de zelfrapportage. Sterker nog, deze misperceptie zorgde ervoor dat de nieuwe stimulus bijna evenveel angst uitlokte als de initiële vreesinducerende stimulus.

Maar welke therapie helpt dan?

Het feit dat angst gegeneraliseerd werd door het onvermogen te kunnen discrimineren tussen verschillende stimuli suggereert een belangrijke rol van perceptie in de generalisatie van vrees. Dit impliceert ook dat bij angstpatiënten een verkeerde perceptie een belangrijke rol kan spelen. Helaas wordt perceptie onderschat in hedendaagse therapieën. Omdat deze studie relatief vernieuwend is, is er nog verder onderzoek nodig op zowel klinisch als neuropsychologisch vlak. Aangezien verschillende mechanismen kunnen resulteren in hetzelfde pathologische gedrag (namelijk angststoornis), is het belangrijk om deze mechanismen aan het begin van de therapie zo goed mogelijk te identificeren zodat er een efficiënte behandeling opgestart kan worden. Angstpatiënten met een cognitieve bias halen bijvoorbeeld meer voordelen uit cognitieve gedragstherapie terwijl angstpatiënten met perceptuele problemen meer baat hebben bij een perceptuele discriminatietaak.
