Epitheliale-Mesenchymale-Transitie bij borstkanker en metastasering

Mijn scriptie Epitheliale-Mesenchymale-transitie (EMT) bij borstkanker en metastasering brengt het proces van kanker en meer bepaald metastase bij borstkanker in kaart.  Deze literatuurstudie geeft de meest recente inzichten weer over EMT toegepast op borstkanker.

Epitheliale-mesenchymale-transitie (EMT)

Epitheliale-mesechymale-tranisitie is een biologisch proces waarbij epitheelcellen omgevormd worden tot mesenchymale cellen. Epitheelcellen of opperhuidcellen bevatten een typische boven en onderkant, ook wel apicaal en basaal genoemd. Deze cellen komen typisch voor ter hoogte van de huid, slijmvlies van de darm en de oppervlakkige cellen van de klierbuizen. Ze hebben de eigenschip om zich aan elkaar vast te klampen en vormen zo een goede barrière tegen  indringers (bacteriële kiemen), wat er voor zorgt dat die kiemen  niet in de bloedbaan kunnen. Een epitheelcel kan in bepaalde omstandigheden omgevormd worden tot  een mesenchymcel. Mesenchymcellen zijn wormvormige cellen die geen typisch apicale en basale vorm hebben.  Verder bevatten mesenchymcellen de eigenschap om vrij rond te migreren in het omgevend weefsel.

Soorten EMT

Er bestaan 3 soorten EMT processen. EMT type 1 is het EMT dat voorkomt tijdens de embryogenese, meer bepaald tijdens de implanting van de bevruchte eicel, de gastrulatie en de verdere orgaan ontwikkeling van het embryo. Men kan begrijpen dat een complex ontwikkelingsproces zoals embryogenese nieuwe celeigenschappen moet ontwikkelen om zo een definitief orgaan te vormen. Het EMT type 2 is de EMT dat voorkomt bij wondheling, weefselherstel en fibrosevorming. Bij wondheling zorgen de nieuw gevormde mesenchymcellen  voor  aanmaak van collageen, een bindweefsel dat een wonde sluit.  Tenslotte is EMT type 3 het EMT dat voorkomt bij kankerprocessen.  Wanneer een kankercel de eigenschap krijgt om van celentiteit te veranderen kan de cel naar de bloedbaan gaan en metastaseren naar verschillende organen.

Borstkanker

Borstkanker is de meest voorkomende kanker bij de vrouw. Één op de tien vrouwen zal ooit in haar leven te maken krijgen met borstkanker. Wanneer borstkanker gemetastaseerd is, is de 5 jaars overlevingspercentage 16% (1/6).  Een vergevorderde borstkanker die nog net niet gemetastaseerd is daarentegen, is de 5 jaars overlevingspercentage 49 % (1/2). Met andere woorden wanneer borstkanker metastaseert, heeft een vrouw 3 keer meer kans om binnen 5 jaar te overlijden. Activatie van EMT in kankercellen zorgt voor metastase. Het opsporen van EMT bij een patiënt met borstkanker zou veel informatie geven omtrent de ernst en prognose van de kanker.

EMT pathways

De 4 belangrijkste EMT pathways betrokken bij borstkanker zijn:

  1. PI3K-AKT-mTOR of PAM pathway
  2. GSK-3b/Wnt cascade
  3. Cel polariteit regulatoren
  4. microRNAs

Deze 4 pathways gaan verschillende EMT transcriptiefactoren zoals Snail, Twist, Slug, ZEB1/ZEB2 activeren. Stimulatie van EMT transcriptiefactoren zorgen voor een repressie of verminderde expressie van het adhesie-eiwit E-cadherine. Dit leidt tot verminderde aanmaak van intercellulaire verbindingen en uiteindelijk loskomen van epitheelcellen.

In deze samenvatting worden de eerste 2 pathways besproken om idee te geven hoe EMT bij borstkanker geactiveerd kan worden.

Normale epitheelcellen zijn met elkaar verbonden aan de hand van “adherens junctions” en “Tight junction”. Adherens junctions zijn intercellulaire verbindingen tussen epitheelcellen die opgebouwd worden door  E-Cadherines. Wanneer EMT geactiveerd wordt, zal de productie van E-Cadherines verminderen en aanleiding geven tot het vrijkomen van epitheelcellen. Hierdoor kan metastase in gang gezet worden.

De PI3K-AKT-mTOR pathway (PAM pathway)

Phosphinositide 3-kinas (PI3K)/AKT/ mamalian target of Rifampicine (mTOR) pathway is een intracellulaire pathway die de celcyclus stimuleert. Wanneer deze pathway in overmaat wordt geactiveerd, leidt dit tot activatie van EMT. De PAM pathway wordt in gang gezet door TGF-b en WNT moleculen. Deze moleculen activeren het AKT eiwit wat op zijn beurt vier andere pathways regelt, bestaande uit mTORC1, mTORC2, NF-kb en GSK-3b. De mTOR eiwitten zorgen voor celgroei en actine reorganisatie. Verminderde GSK-3b zorgt voor een verminderde expressie van E-cadherine.

GSK-3b/Wnt pathway

De glycogeen synthase kinase 3 beta/Wnt pathway staat in voor cel proliferatie en cel migratie. De PAM pathway stimuleert de GSK-3b/Wnt pathway. Dit gebeurt door inhibitie van GSK-3b eiwit wat leidt tot activatie van de Wnt pathway.

Wnt pathway wordt geactiveerd door een binding van het Wnt ligand op Frizzled receptor (Fzd). In de inactieve toestand zijn er geen Wnt liganden waardoor de Frizzled receptor niet geactiveerd wordt. In deze inactieve toestand wordt een multi-subunit destructie complex gevormd. Dit multli-subunit destructie complex zorgt  voor afbraak van gefosforyleerde b-catenine. Wanneer de Frizzled receptor geactiveerd wordt, zal het Disheveled (DSH) eiwit in het cytoplasma vrijkomen en binden op GSK-3b. Daarzoor zal het GSK-3b eiwit niet functioneren en geen destructiecomplex vormen. Bijgevolg komt het b-catenine vrij en transloceert naar de nucleus, waar het intraheert met de transcriptiefactor TCF/LEF, wat leidt tot transcriptie van c-myc, cyclin D en het activeren van transcriptiefactoren. Wnt zorgt voor verminderde expressie van E-cadherine.

Samengevat zal EMT activatie de PAM pathway activeren waardoor er minder E-cadherine wordt vrijgesteld en cellen loskomen. Verder activeert de PAM pathway de GSK-3b/Wnt pathway waardoor er celmigratie en celgroei ontstaat.

Besluit

Epitheliale-Mesenchymale Transitie (EMT) is een biologisch proces dat een rol heeft in het ontstaan van metastase.  Metastase van borstkanker ontstaat door EMT, waarbij de epitheelcellen omgevormd worden tot mesenchymcellen en kunnen uitzaaien.  EMT vindt plaats door activatie van verschillende pathways zoals de PAM pathway en GSK-3b/Wnt pathway. Het beter begrijpen van de verschillende pathway’s van EMT en metastase kan leiden tot specifieke therapeutische doeleindes.  Bijvoorbeeld, een blokker die specifiek op één van de pathway’s van EMT inwerkt, kan mogelijks metastase bij borstkanker afremmen.
