Braken en de symptomatische behandeling van onze blaffende viervoeters

Iedereen die ooit baasje is geweest van een hond, kent het gemak waarmee honden hun maag kunnen ledigen, en dit gebeurt vaak op het meest ongepaste moment! Er kunnen legio triggers zijn wat leidt tot braken in de hondenwereld. Of het nu het resultaat is van het opeten van een gebraden kip, dan wel opgewonden worden door de aanblik van de passerende oma met haar twee Yorkshire Terriërs, er zijn meer oorzaken die kunnen leiden tot braken dan u denkt.

Ongeacht het feit dat veel hondenbaasjes hun viervoeters goed kunnen verzorgen, is braken niet altijd te voorspellen en huis-, tuin- en keukenmiddelen hebben hun beperkingen. Dierenartsen zijn eerstelijns bij “braak”ongevallen. Dierenartsen zijn zich er van het begin af aan van bewust dat zulke gevallen een wisselvallig verloop kunnen hebben. Vaak betreft het een enkelvoudige gastritis, maar als het complexer is zou dit mogelijk het dagelijks leven van hond en baas voorgoed kunnen veranderen.

Het is voor de dierenarts van groot belang om te weten wat de hond precies heeft gedaan tot het moment van braken. Op basis van de voorgeschiedenis van de hond bepaalt de dierenarts welke acties ondernomen moeten worden. Word ik geconfronteerd met een eenvoudig maagprobleem, of zijn er subtiele aanwijzingen dat er met deze hond meer aan de hand is? Braakt onze Fifi of heeft ze daadwerkelijk een probleem met slikken? En hoe gênant dit misschien ook klinkt, het is van groot belang om te weten of Snoepie uw speciale tabak heeft opgegeten of niet. Dit is niet alleen vanwege uw hond maar ook vanwege uw portemonnee. Een diagnose is als het oplossen van een puzzel. Alle informatie die u geeft en de symptomen die de hond vertoont kunnen leiden tot de oplossing van de puzzel.

Ondanks de vooruitgang in de moderne (dier)geneeskunde, dat de job van de dierenarts steeds makkelijker maakt, is er helaas niet één medicijn die alle vormen van braken kan behandelen. Dit komt door de verschillende orgaansystemen van het lichaam die elk op hun eigen wijze reageren. Receptoren in elk systeem detecteren veranderingen ten opzichte van de norm. Bepaalde receptoren in de maag reageren op specifieke medicijnen, en deze receptoren kunnen verschillen van die in het binnenoor. Deze laatste detecteert wisselingen in het evenwicht. Denk bijvoorbeeld aan de mens die een vistocht gaat maken met windkracht 8. Het spreekt voor zich dat de inname van een dosis maagzuurremmers je niet van het onvermijdelijke kan redden...

Dierenartsen maken gebruik van hun kennis van de braakreflex en de betrokken receptoren om de systemen in het lichaam te manipuleren en om zo onze kotsende patiënt te helpen. Helaas is het onderdrukken van de receptoren alleen niet voldoende als de oorzaak ook niet behandeld wordt. Bijvoorbeeld braken veroorzaakt door een infectie met gastro-intestinale wormen kan niet gestopt worden zonder de gelijktijdige uitroeiing van de dader (de wormen). Hoe gaan dierenartsen aan de slag wanneer ze geconfronteerd worden met een brakende patiënt?

De behandelingskeuze kan vaak afgeleid worden uit de symptomen en de voorgeschiedenis van de hond. Als we weten dat de gezonde hond alleen braakt tijdens een autorit, dan kan een dierenarts voorlopig kiezen om anti-emetica voor reis-ziekte te geven. In dit geval is de voorgeschiedenis de autorit, echter in andere gevallen is dit niet altijd even duidelijk en kunnen verdere diagnostische testen noodzakelijk zijn. Deze testen kunnen veel van de vermoedelijke oorzaken uitsluiten en de dierenarts helpen om de patiënt te stabiliseren. De kern van mijn masterproef is gericht op dit punt – de symptomatische behandeling van de brakende hond. In de vroege diagnostiek, is het behandelen van de symptomen vaak belangrijker dan het zoeken naar de oorzaak. In mijn masterproef wordt de basisbehandelingen in detail vergeleken en hoe homeostase bereikt kan worden. Dit wordt bereikt door het gebruik van nutritionele-, ondersteunende- en medicinale therapie.
