342. Het aantal kinderen dat in 2013 een kankerdiagnose kreeg. Jaarlijks krijgen een 300-tal kinderen en gezinnen te maken met de woorden: ‘meneer/mevrouw, uw zoon/dochter heeft kanker. Woorden die een grote impact hebben. Woorden die meteen voor diverse gevoelens zorgen: teleurstelling, woede, verdriet, hoop en wanhoop, rusteloosheid, angst … En dan, het kind wordt vrijwel meteen na de diagnose opgenomen in het ziekenhuis om te starten met de behandeling. Een behandeling die vaak lang en intensief is. Kinderen worden geconfronteerd met pijn en een onbekende wereld. En naast die onbekende wereld, wordt hun eigen gekende en vertrouwde wereld ook nog eens verstoord. Kinderen mogen niet meer naar school, kunnen niet meer eten wat ze willen, krijgen voortdurend medicatie en spuitjes, kunnen hun hobby’s niet meer uitvoeren, worden gescheiden van hun ouders, broers/zussen, grootouders, vriendjes … Kortom, de gevolgen van een kankerdiagnose zijn groot, zowel fysiek als emotioneel, en zeker niet te onderschatten.

Echter, welke de psychologische gevolgen zijn op lange termijn is een onderwerp die niet altijd even sterk aan bod komt in de literatuur. In mijn masterproef wou ik daarom nagaan welke impact de diagnose had op lange termijn op drie gebieden: distress, algemeen welzijn en psychologische flexibiliteit. Dit werd nagegaan bij zowel de zieke kinderen als hun gezonde broers/zussen. De data werden verzameld in het kader van het doctoraatsonderzoek van Marieke Van Schoors: “Families and child cancer. A study on crisis, resources and adaptation’. In dit doctoraat focust men op de diagnose kinderkanker en de ruimere invloed hiervan op het gezin (zowel ouders, patiënt als broers en zussen). Dit onderzoek is nog steeds lopende waardoor het aantal deelnemers voor de masterproefstudie beperkt bleef. Een belangrijke en opvallende bevinding vonden we bij de zieke kinderen: nl. dat distress stijgt over tijd. Bij distress kijken we in hoe verre men zichzelf adequaat acht om met de ziekte om te gaan. Uit de eerder verschenen literatuur verwachtten we dat hoe meer tijd verstreken is sinds de diagnosestelling, het kind beter kan omgaan met stresserende gebeurtenissen en dus minder distress vertoont. Echter, in het masterproefonderzoek werd het omgekeerde gevonden: nl. hoe verder van diagnosestelling, hoe meer distress en hoe moeilijker het wordt om met de ziekte om te gaan.

Wanneer we dit bespreken met ouders en kinderen, dan erkennen zij dat de stress vaak na enkele maanden pas een piek bereikt. De ziekte wordt immers vaak ervaren als iets dat je overvalt in het begin. Men wordt meegezogen in het gebeuren, een rollercoaster die het gezin meeneemt. Later, wanneer de behandeling minder wordt, en op zijn einde loopt, wordt de realiteit pas echt duidelijk. Mensen leven vaak maanden in een ziekenhuiscocon, en plots zijn ze terug thuis. Het gewone ‘oude’ leven begint terug. Dit is vaak het moment waarop de realiteit van de voorbije maanden doordringt en het besef voor meer angst en onzekerheid zorgt. Dit alles wijst erop dat het nuttig kan zijn om psychologische begeleiding te voorzien lang na diagnosestelling en dus na de intensieve behandeling. Kinderen helpen bij de herintegratie in het ‘gewone’ dagdagelijkse leven zou van grote waarde kunnen zijn. Deze bevinding is slechts summier en moet hertest worden op grote schaal. Het toont echter wel aan hoe belangrijk psychologische hulp kan zijn op lange termijn, en laat deze studie hopelijk een opstap zijn naar meer psychologisch onderzoek over de impact van kinderkanker op lange termijn, zowel bij het zieke kind als het volledige gezin
