Welke gevoelens en emoties komen voor bij psychiatrische patiënten opgenomen op een intensieve zorgafdeling na een isolatie of fixatie?

Stel u voor: je voelt je al een tijdje minder goed. Je begint slecht te slapen en het is moeilijk om dagdagelijkse taken te volbrengen. Daarbovenop komt dat je plots stemmen hoort in je hoofd, hiermee ga je naar de spoedgevallendienst van het algemeen ziekenhuis en wordt je doorverwezen naar een afdeling in een psychiatrisch ziekenhuis. Je gaat in opname maar plots zeggen de stemmen dat het niet veilig is in het ziekenhuis, dat “ze” het op je gemunt hebben. Vervolgens ga je er alles aan doen om weg te kunnen. Dit wordt je niet toegestaan. Hierdoor ben je agressief en vijandig. Het volgende dat gebeurd is dat je erg beperkt wordt in je doen en laten. Dit is niet meteen wat je verwacht had van de behandeling. Gelukkig zijn de stemmen meer op de achtergrond en kom je stilaan terug in de realiteit…

In deze scriptie wordt getracht een antwoord te geven op de vraag: welke gevoelens en emoties komen voor bij psychiatrische patiënten opgenomen op een intensieve zorgafdeling na een isolatie (verblijf in een prikkelarme kamer) of fixatie (vrijheid beperkende maatregel d.m.v. fixatiemateriaal aan het bed)? Deze scriptie is opgebouwd uit 3 grote delen: een literatuurstudie, een zelf gevoerd onderzoek op de afdeling voor intensieve psychiatrische zorg (IPZO) in het psychiatrisch ziekenhuis Stuivenberg te Antwerpen. Deze resultaten worden geïmplementeerd in de reeds bestaande bijscholing “isolatie – en fixatietechnieken”.

Voor de literatuurstudie is er gebruik gemaakt van 25 nationale en internationale wetenschappelijke artikelen.

Het eigen onderzoek is gebaseerd op een patiëntinterview dat afgenomen werd nadat patiënten een dwangmaatregel hebben gehad. De onderzoekspopulatie bedraagt 10 patiënten.

In het onderzoek zijn geen baanbrekende elementen naar voren gekomen. Wel is naar voren gekomen dat enkele aanbevelingen in de literatuur, zoals het duidelijk communiceren van de reden van opstart van de dwangmaatregel en de te verwachten doelstellingen naar de patiënt toe, in onvoldoende systematische mate gebeurt.

Betreffende de gevoelens en emoties van patiënten na het gebruik van dwangmiddelen in het psychiatrisch ziekenhuis wordt de rollercoaster van gevoelens en emoties (hoofdzakelijk negatieve zoals beangstigend, schaamte, gevoel van straf) bevestigd in dit onderzoek.

Patiënten zijn negatiever over fixatie dan over separatie (zo veilig mogelijke  kamer met de deur gesloten) en negatiever over separatie dan over noodmedicatie (extra medicatie in opdracht van psychiater)

Het spreekt voor zich dat deze resultaten met de nodige kritische blik moeten bekeken worden. Zo is de onderzoekspopulatie erg klein en zijn er mogelijke factoren die een invloed uitoefenen waarmee geen rekening werd gehouden zoals bv. situationele factoren. Deze kritische blik geldt eveneens voor de literatuurstudie vermits één van de conclusies is dat het zeer moeilijk is om goed gevoerde, representatieve en onderling vergelijkbare onderzoeken te vinden in de beschikbare databases.
