‘An app a day, can it keep the nutritionist away’, onderzoek naar de gelijkenissen, verschillen en toekomst van apps binnen een diëtistenpraktijk

De opmars en stijgende populariteit van mobiele applicaties, beter bekend onder de afkorting apps, is ook voelbaar in de wereld van diëtiek. Iedereen kent wel iemand die praat over een ‘digitale’ diëtist of die de maaltijden in een app plaatst. Maar hoe zit het met de kwaliteit van deze apps en zijn er linken met de werking van een ‘klassieke’ diëtist?

De onderzoeksvragen die centraal stonden binnen deze bachelorproef focusten zich dus op bovenstaande probleemstelling. Als eerste werd nagegaan of de werking van deze apps gelijk is aan de werking van een ‘klassieke’ diëtist. Vervolgens werden de verschillen tussen beiden ook grondig bekeken en genoteerd. De laatste onderzoeksvraag was of deze twee elkaar kunnen verrijken en indien ja, hoe zou deze verrijking dan gebeuren?

De hoofddoelstelling was dus de kwaliteit van apps rond voeding en dieet nagaan. De nadruk binnen deze bachelorproef lag op applicaties die focusten op een energiebeperkt dieet. Hoewel er een groot aanbod is van apps die ingezet kunnen worden bij pathologieën, werden deze niet opgenomen. Als extra doelstelling werd bekeken in welke mate ze inzetbaar zijn in de huidige diëtistenpraktijk. De toekomst van apps binnen deze praktijk werd daarbij ook onder loep genomen. Tot slot vormden al deze zaken de basis tot het ontwikkelen van een opsomming van zaken waaraan de ‘ideale’ app moet voldoen.

Het materiaal dat vereist was, was uiteraard een smartphone met bijhorende applicaties. De gebruikte smartphone bij deze bachelorproef was een iPhone 4. Het aantal apps dat geselecteerd werd waren 11 stuks. De beoordeling van de applicaties gebeurde aan de hand van checklist opgesteld uit de literatuur. De checklist bevatte 29 factoren verkregen uit de grondige literatuurstudie. Deze vatten samen aan welke zaken een goed diëtistisch consult moet voldoen. Op die manier werd vooral de kwaliteit van de apps vergeleken met de kwaliteit van een consult bij de diëtist.

De apps werden geselecteerd op basis van bepaalde vooropgestelde criteria. Deze waren noodzakelijk aangezien het overaanbod aan apps nood heeft aan wat structuur. De geselecteerde apps moesten aan minimum één van deze criteria voldoen. De app moest behoren tot een vaak gedownloade, populairdere groep. De vermelding van de app in literatuur of (wetenschappelijke) artikels was een meerwaarde. Tot slot was er de afwezigheid van apps die puur focusten rond fitness en beweging, de combinatie van voeding en beweging werd wel geaccepteerd.

Alle apps werden gedownload uit de Appstore van Apple en waren gratis. Dit omdat de literatuur vermelde dat betalende applicaties geen meerwaarde boden ten opzichte van de gratis varianten.

Het uittesten van de applicaties zelf werd ook volgens een vaste structuur gedaan. Zo vond eerst de omschrijving plaats. Dit was het verkooppraatje van de producenten welke online of bij de installatie van de app te vinden was. De evaluatie zelf gebeurde aan de hand van de checklist door middel van afvinking. Vervolgens vond de sterkte-zwakte analyse plaats. Hierbij werden de positieve als negatieve factoren ten opzichte van elkaar afgewogen en gekeken wat de voor- als nadelen zijn voor zowel de diëtist als de cliënt die deze app gebruikt. Het besluit vatte bovenstaande zaken samen tot een samenhangend geheel en vermelde of de app al dan niet aan te raden viel.

Wanneer we de cijfers uit het onderzoek vergelijken met de literatuurstudie kunnen de grootste verschillen tussen apps en diëtisten genoteerd worden.

×           Kennismaking

Er is altijd navraag naar de lengte, gewicht, geslacht en geboortedatum van de gebruiker. Soms wordt de mate van fysieke activiteit nagevraagd en slechts zelden komen de smaakvoorkeuren, gebruikelijke portiegroottes als bereidingswijzen van de gebruiker aan bod.

 ×           Motivatie en steun

Er wordt vanuit gegaan dat iedereen die een app download een hoge motivatie heeft. Diëtisten weten daarentegen dat niet elke patiënt even gemotiveerd is tijdens de consultaties. 

Door het gebrek aan kennismaking, motivatie en steun kan gezegd worden dat het persoonlijk karakter van de apps op dit moment ondermaats is.

 ×           Educatie

Er is minimaal educatie aanwezig in apps. Zo weten personen zonder enige voorkennis niet hoe etiketten gelezen moeten worden, hoe veilig te sporten, of hoe een gezonde maaltijd moet samengesteld worden. Een andere grote afwezige was de actieve voedingsdriehoek. Deze kon in geen enkele applicatie terug gevonden worden, hoewel deze de basis vormt van een goed diëtistisch consult.

 ×           Leren plannen van sport

De meeste apps maken gebruik van een bewegingsdagboekje waarin de gebruiker de gepresteerde activiteiten kan plaatsen. Een belangrijke afwezige is hoe deze activiteit kan ingepland worden, hiervoor is de gebruiker op zichzelf aangewezen.

 ×           Energieberekening

De energieberekeningen zijn nauwelijks of zelfs helemaal niet wetenschappelijk onderbouwd. Het gebruik van een algemene formule is vaak van toepassing. Bij sommige apps wordt echter helemaal geen formule vermeld en een standaardcaloriebehoefte meegedeeld.

Het ontwerpen van een ‘ideale’ app zou dus meer aanleunen bij het werk dat een diëtist(e) uitvoert. Enkele zaken die deze applicatie zou bevatten:

Het inschakelen van een diëtiste bij de ontwikkeling waardoor de betrouwbaarheid toeneemt in combinatie met een toename van de motivationele factoren. Het eetdagboek kan ook wat aanpassing gebruiken. Idealiter zou een fotodagboek hiervoor de perfecte oplossing zijn. Hoewel deze technologie al in ontwikkeling is, hangt er uiteraard een groot prijskaartje aan vast. Samenwerken met een goede levensmiddelendatabank en aldus betrouwbare database (vb. Nubel of Nevo) zou een slimme stap zijn.

Ook is er nood aan een correctere energieberekening. Deze berekening begint idealiter bij een ruimere vraagstelling en is gebaseerd op wetenschappelijke kennis en achtergrondinformatie. Tot slot zou de app enkel voor cliënten zijn die al op consultatie komen.

Als besluit kan vermeld worden dat apps nog niet gelijk zijn aan de werking van een diëtist. Modernisering van de praktijk is een stap die vroeg of laat gezet zal moeten worden. Jongeren van vandaag zijn nu eenmaal de ouderen van morgen. Concreet zal de toekomst bepaald worden door de diëtisten zelf. Zij bepalen nu eenmaal of deze apps ingezet zullen worden of dat het slechts bij een kortstondig succes blijft.
