De epidemie raast verder

Tuberculose (tbc) is terug van nooit weggeweest; in 1993 werd het door de World Health Organisation uitgeroepen tot een globaal noodgeval. Het is nog steeds de op één na dodelijkste infectieziekte (na AIDS), waarbij vooral de jonge laag van de maatschappij getroffen wordt. De bacterie in kwestie, Mycobacterium tuberculosis, is moeilijk uit te roeien wegens het enorme reservoir van mensen met een volhardende, latente tbc infectie (ongeveer 1/3 van de wereldbevolking). Factoren zoals migratie en sociale ongelijkheid maken het er niet makkelijker op. Deze ziekte is voor meer dan 40 jaar aan onze aandacht ontsnapt door onze eigen hoogmoed. Intensief onderzoek leidde tot efficiënte therapieën die het voorkomen van tbc drastisch verlaagde in de ontwikkelde landen, maar dit weerhield M. tuberculosis niet van hard toe te slaan in de ontwikkelingslanden door het gebrek aan gezondheidszorg, ondervoeding en de recente HIV pandemie.

De familie van Mycobacteriaceae, waartoe M. tuberculosis behoort, is een verzameling bacillen gekenmerkt door een zeer hoge inherente resistentie (tegen oa. geneesmiddelen) en persistentie. Om tuberculose te behandelen is dus momenteel een zware, lange therapie (tot 2 jaar lang naargelang de complicaties) nodig. Deze kent veel nevenwerkingen door de lange duur en het aantal geneesmiddelen dat men moet nemen (in sommige gevallen zelfs 4 of meer!!). Deze nachtmerrie op het vlak van therapietrouw achtervolgt ons nu met de opkomst van resistente stammen van tbc, waarvoor we onze toevlucht moeten nemen tot steeds gevaarlijkere producten.

Niet veel nieuws onder de zon! Of toch wel?

Het huidig ontwikkelingsvooruitzicht voor nieuwe antimycobacteriële middelen is bijzonder mager. Zo worden oude geneesmiddelen (bv. fluorochinolonen) met gekende activiteit nu getest voor tuberculose als nieuwe indicatie. Verder worden nieuwe stoffen ontwikkeld vanuit bestaande medicaties (bv. metronidazole, linezolid) in de hoop ze voorbij de weinig permeabele celwand van M.tuberculosis te krijgen.

Maar de ontdekking van een nieuwe manier om de synthese van thymidine - één van de vier DNA bouwstenen - te inhiberen, biedt ons nieuwe aangrijpingspunten voor de ontwikkeling van nieuwe antibiotica. Centraal hier is het flavine afhankelijk thymidylaat synthase (FDTS). Dit enzyme is aanwezig in verscheidene organismen inclusief enkele menselijke ziekteverwekkers zoals S. typhi, B. anthracis en M. tuberculosis. Daarbovenop werd er aangetoond dat FDTS noodzakelijk is voor optimale groei zelfs in micro-organismen die zowel FDTS als het klassieke, oa bij de mens voorkomende, thymidylate synthase (classical TS) gebruiken. De eerste vergelijkende structurele studies gaven voldoende aanwijzingen over de structurele en mechanische verschillen, om FDTS te poneren als veelbelovend nieuw en selectief doelwit.

Het is daarom dat het labo van Medicinale Chemie zich inzet in een multidisciplinair onderzoek naar de ontwikkeling van antibiotica gebaseerd op de inhibitie van FDTS. Moleculen met geringe activiteit, ontdekt door het testen van grote verzamelingen van molecuulbibliotheken, worden ingezet voor verdere opheldering van de structuur en het werkingsmechanisme van FDTS. Dit laat toe de gevonden moleculaire structuren gericht aan te passen aan de noden van nieuwe antibiotica zoals hoge activiteit en selectiviteit.

Fragment based drug discovery

Screening van nieuwe moleculen gebeurt tegenwoordig steeds met kleinere fragmenten. Zo blijft er meer ruimte over voor optimalisatie aangezien de grootste hindernissen van de huidige geneesmiddelenontwikkeling niet liggen in het vinden van actieve, maar wel in het vinden van moleculen met de nodige biologische beschikbaarheid, zodat er voldoende geneesmiddel wordt opgenomen door het lichaam en men een voldoende hoge concentratie krijgt waar de tbc infectie plaatsvindt. Dit heeft als gevolg dat er meer producten gevonden worden maar met geringe activiteit. Verdere ontwikkeling gebeurt dan door fragmenten aan elkaar te linken of uit te bouwen.

Aan de wieg van een nieuw antibioticum

Na het testen van verscheidene commercieel beschikbare producten, kwam een veel belovende reeks van tetrahydrocarbazolen naar voren (Tabel 1). Deze verbindingen zijn al reeds goed gekend en vormen al de basis van verscheidene andere geneesmiddelen. Het doel van deze thesis was het uitbreiden van de bibliotheek met analoge verbindingen gebruik makend van een in de vergetelheid geraakte synthese methode (Bischler Indole synthese). Hoewel deze reactie lage opbrengsten geeft, komt deze toch terug op de voorgrond door zijn relatieve eenvoud in gebruik zowel qua geïnvesteerde moeite als qua benodigde materialen. Dit laatste element krijgt steeds meer belangstelling in de zoektocht naar ‘green chemistry’. Het groeiend milieubewustzijn van de publieke opinie dwingt de hedendaagse scheikundige zijn creativiteit te richten naar het verminderd gebruik van zware metalen en moleculen afkomstig van olie-producten als reagentia. Alhoewel ik moet toegeven dat naast het milieubewuste, een stijgende olieprijs er ook iets mee te maken heeft.

Het gebruik van verschillende methodes om dezelfde reactie uit te voeren heeft interessante informatie opgeleverd over de reactie zelf. Deze werd toegepast om de opbrengst te verhogen gaande van aanvaardbaar tot zelfs zeer hoog. Na de op puntstelling, werden een twintigtal analoge verbindingen gesynthetiseerd, gekarakteriseerd en getest voor hun potentieel als inhibitor voor FDTS. Het beschikken van een reeks verbindingen die weinig verschillen qua bouw maar met significante activiteitsverschillen stelt ons in staat om conclusies te trekken en van daaruit de verdere ontwikkeling van deze moleculaire kern te sturen. Verscheidene voorstellen om dit aangrijpingspunt verder te ontwikkelen werden voorgesteld in de afsluitende literatuurstudie (het tweede gedeelte van deze masterthesis). Zo zullen de ontwikkelde syntheseprotocols kunnen gebruikt worden voor de synthese van intermediairen, ideaal om te linken aan andere nog te ontdekken actieve fragmenten.

Conclusie

De strijd tussen gastheer en pathogeen is één van lange adem en wij zijn, verblind door hoogmoed, in snelheid genomen door het aanpassingsvermogen van tbc. Maar er is verandering op komst vanuit zowel ngo’s als TBAlliance.org als de academische wereld. Zo draagt deze thesis zijn steentje bij in het moeizame onderzoek naar antibiotica en, specifiek, in het onderzoek naar nieuwe middelen tegen tuberculose.
