Studie van de rol van het VEGFA gen in de oogsymptomen bij Pseudoxanthoma Elasticum

Voorspellen van Oogproblemen: Fictie of realiteit?

Pseudoxanthoma Elasticum (PXE) is een zeldzame erfelijke aandoening waar tussen 1/25.000 en 1/100.000 mensen aan lijden. De meest frequent aangetaste lichaamsdelen zijn de ogen, de huid en het hart- en bloedvatenstelsel. Hoewel al veel vooruitgang is geboekt in de ontrafeling van dit ziektebeeld, zijn er nog steeds veel vraagtekens over het ontstaan en ziekteverloop van PXE; zo is genezing nog lang niet mogelijk en kan slechts geprobeerd worden om de symptomen aan te pakken wanneer deze zich voordoen. Het gevolg is dat artsen én patiënten vaak het gevoel hebben dat ze achter de feiten aanlopen, met alle frustraties van dien. Een recent onderzoek wilde hier dan ook verandering in brengen, althans voor één van de aangetaste delen van het lichaam: het oog. Kunnen we voorspellen wie zware oogproblemen zal ontwikkelen? En nog veel belangrijker, kunnen wij patiënten behoeden voor deze symptomen? Uit de resultaten van dit onderzoek kunnen we voorzichtig ja zeggen. Verder bevestigend onderzoek is reeds gepland  en zal voor definitieve zekerheid zorgen.

Pseudoxanthoma elasticum vertoont een enorme verscheidenheid aan problemen of symptomen. Deze beginnen zich al te ontwikkelen op kinderleeftijd en worden steeds ernstiger. Het gaat met andere woorden om een chronische aandoening, zoals diabetes (suikerziekte) en een te hoge bloeddruk. Ter hoogte van de huid ontstaan gelige verhevenheden, het hart en de bloedvaten vertonen versnelde verkalking en de oogproblemen kunnen leiden tot heel ernstige beperkingen van het zicht. 

PXE wordt veroorzaakt door mutaties in een welbepaald gen (ABCC6 gen). Mutaties zijn ernstige fouten die gelegen zijn in een gen en die meestal ziekte veroorzaken. Een gen op zijn beurt is gelegen op een chromosoom. Het lichaam bestaat uit 46 chromosomen, 22 paar en 2 geslachtschromosomen. Van elk gen dat niet gelegen is op de geslachtchromosomen, zijn dus sowieso 2 kopieën aanwezig, één afkomstig van de moeder en één van de vader. Om PXE te ontwikkelen moet een fout  aanwezig zijn in het gen afkomstig van de moeder én in het gen afkomstig van de vader. Men spreekt van een recessieve aandoening. Mutaties zijn ook verantwoordelijk voor ziektebeelden zoals mucoviscidose (taaislijmziekte) en  hemofilie (ernstige bloedziekte).

Het ziektebeeld PXE vertoont een zeer grote variabiliteit wat betreft de ernst van aantasting van de verschillende lichaamsdelen; zo vertonen sommige patiënten heel ernstige afwijkingen, terwijl anderen een weinig ingrijpende aantasting vertonen, en dit zelfs binnen eenzelfde familie! Bij PXE zijn veel verschillende mutaties in het ABCC6 gen mogelijk en hoewel deze weliswaar de oorzaak van het ziektebeeld PXE zijn, kon geen relatie gevonden worden tussen welbepaalde mutaties en de ernst van evolutie van de symptomen Dit bezorgt zowel patiënten als artsen heel wat frustraties aangezien het dus nattevingerwerk is te bepalen bij wie de ziekte snel dan wel traag zal evolueren.

PXE is een zogenaamde ‘orphan disease’. Het is een zeldzaam weesje – wees dus niet verbaasd als u nog nooit van PXE gehoord hebt – waarnaar, in vergelijking met ziektebeelden als HIV en kanker, slechts een beperkt aantal onderzoeken wordt gevoerd. Dit zorgt ervoor dat bij PXE vele vragen nog steeds onbeantwoord blijven. Zo weet men wel welke lichaamsdelen het meest worden aangetast, maar niet precies waarom het net deze zijn. Ook is het nog niet mogelijk om te genezen van de ziekte. Men kan wel trachten het ziektebeeld te vertragen of sommige symptomen voor te zijn en dus al te behandelen alvorens grote problemen ontstaan. Een extra moeilijkheid hierbij is de tot nog toe onverklaarbare variabiliteit in de symptomen.

Recent onderzoek focuste zich op één van de aangetaste lichaamsstelsels: het oog. Het doel was duidelijk: is er een mogelijkheid om de variabiliteit in oogproblematiek op een wetenschappelijke manier te verklaren en kan dit in de praktijk worden gebruikt? Met andere woorden: kunnen we oogsymptomen voorspellen aan de hand van het DNA van de patiënt?

Hiervoor werd een ander gen onderzocht, het VEGFA gen. Dit gen zorgt voor de aanmaak van een groeifactor die bloedvaten sneller doet groeien. Eén van de ingrijpende symptomen van PXE in het oog is namelijk dat nieuwe bloedvaten kunnen ontstaan achter het netvlies. Deze bloedvaten zijn echter van slechtere kwaliteit dan de originele bloedvaten en gaan gemakkelijker scheuren, wat leidt tot bloedingen en verlies van het zicht. Wanneer men echter op tijd kan ingrijpen, dit is vóór het ontstaan van bloedingen, kan het zicht bewaard blijven. De behandeling bestaat uit inspuitingen in het oog met een anti-VEGF product.

Het VEGFA gen werd, net zoals het ABCC6 gen, onderzocht op veranderingen. Deze keer ging de aandacht echter niet naar mutaties maar naar SNPs (single nucleotide polymorphisms). Dit zijn minieme variaties in het DNA, die niet noodzakelijk ziekmakend zijn, maar onder meer een bijdrage leveren aan de (normale) verschillen tussen mensen onderling. Een groep van 66 PXE-patiënten van het UZGent en het CHU Angers (Frankrijk) werd in twee groepen opgesplitst met respectievelijk ernstige en milde oogsymptomen. Het DNA van de patiënten werd onderzocht op variaties in de groeifactor en een aantal ervan konden via statistische methoden betrouwbaar gekoppeld worden aan ernstigere oogproblemen. Het is nu dus mogelijk om bij alle PXE patiënten de variaties te onderzoeken en op deze manier patiënten onmiddellijk in de ernstige dan wel de milde groep onder te brengen. Patiënten die worden ingedeeld in de ernstige categorie zullen strenger worden opgevolgd, met toename van controles bij de oogarts en zelfcontroles en met het sneller overgaan tot behandeling. Patiënten uit de milde groep worden op de klassieke manier opgevolgd. Na een aantal jaren kan deze manier van opvolging worden geëvalueerd en kan men nagaan of de ernstige groep inderdaad zijn zicht beter behoudt, en of de milde groep zich inderdaad als mild blijft gedragen of dat dit na verloop van tijd kan veranderen.

Ondertussen wordt zowel verder onderzoek gevoerd naar andere genen die mogelijks een rol spelen in de evolutie van oogproblemen als naar de aantasting van andere lichaamsdelen door PXE.
