De ontwikkeling en validatie van een kennistest omtrent psychofarmaca bij verpleegkundigen op de dienst acute geriatrie

 

Kennis van verpleegkundigen: een belangrijke schakel in het overmatig gebruik van psychofarmaca bij ouderen.

Psychofarmacagebruik of het gebruik van o.a. slaap- en kalmeermiddelen scheert hoge toppen onder de Belgische bevolking. In vergelijking met andere Europese landen worden in België gemiddeld het meest psychofarmaca ingenomen. Het hoge gebruik wordt daarbij vooral opgemeten onder de oudere bevolkingsklassen. In dit complexe proces waarbij psychofarmaca worden voorgeschreven en onderhouden, wordt de rol van verpleegkundigen uitbelicht. Dankzij hun centrale rol in het zorgverleningproces, moet de vraag gesteld worden of verpleegkundigen over voldoende kennis en de juiste attitude beschikken om dit hoge psychofarmaca gebruik te legitimeren.

Dankzij een hogere welvaart en medische vooruitgang stijgt de gemiddelde leeftijd elk jaar. Door deze vergrijzing zal ook het aantal ouderen toenemen met een chronische of langdurige ziekte, waarvoor vaak een langdurige medicamenteuze aanpak nodig is. Daarnaast zal de oudere over meerdere ziektebeelden beschikken, waardoor hij niet ongewoon meerdere geneesmiddelen zal moeten innemen. Deze combinatie van polypathologie en polyfarmacie kan in het oudere lichaam een bedreiging zijn. Allereerst daalt de mogelijkheid om een geneesmiddel op effectieve wijze te verwijderen uit het lichaam, waardoor het medicament langer werkzaam zal zijn. Anderzijds zal de kans op interacties tussen geneesmiddelen ook toenemen, naarmate men meer medicamenten slikt.

Uit de PHEBE (Prescribing in Homes for the Elderly in Belgium)-studie uit 2005 blijkt dat maar liefst 79% van de populatie in woon- en zorgcentra minstens één psychotroop geneesmiddel neemt. Praktisch 54% van de populatie in een WZC neemt dagelijks een slaap- of kalmeringspil en 33% een antipsychoticum. Dit hoge gebruik wordt als zorgwekkend omschreven moest met deze geneesmiddelengroepen niet onbedachtzaam omgesprongen worden. Immers, wetenschappelijke richtlijnen raden aan dat deze medicatie slechts gedurende een korte tijd mag ingenomen worden, om vervolgens zorgvuldig af te bouwen. In de praktijk echter ziet men dat deze medicatie vaak jarenlang onderhouden worden bij ouderen. Dankzij hun sederende eigenschappen wordt deze medicatie ook aangewend bij de zware zorg voor patiënten met dementie, waar agressie, onrust of storend gedrag niet ongewoon is.

Het gebruik van psychofarmaca bij ouderen dient eveneens bedachtzaam te gebeuren, maar wordt vaak te lang onderhouden en te snel voorgeschreven. Enerzijds is er slechts een bewezen matige effectiviteit betreffende de behandeling van aandoeningen. Anderzijds wordt deze medicatie meer en meer gecorreleerd met ernstige bijwerkingen. Studies maken gewag van bijwerkingen bij antipsychotica gebruik lijkend op de symptomen bij de ziekte van Parkinson en een gevoelig verhoogde kans op hartaanvallen. Hypnosedativa (waaronder slaapmedicatie valt) is dan weer gecorreleerd met concentratie- en geheugenproblemen. Ook door de verlengde werking in het oudere lichaam stijgt de kans op vallen, door sufheid en gedaalde aandacht, met een verhoogde kans op heupfracturen tot gevolg.

In een poging het proces van langdurig onderhouden van deze medicatie te begrijpen, wordt de rol van verpleegkundigen onder de loep genomen. Zij worden geconfronteerd in hun zorg voor patiënten met dementie met o.a. gedragsstoornissen, storend of agressief gedrag. Dit maakt dat verpleegkundigen vaak zelf vragende partij zijn een therapie op te starten, zonder de effectiviteit of bijwerkingen te kennen van deze medicatie. Doordat verpleegkundigen een rol kennen in zowel het voorschrijven, het onderhouden en het afbouwen van medicatie, rees de vraag of verpleegkundigen over voldoende kennis beschikken om een optimale zorg te waarborgen.

Deze vraag werd beantwoord via een onderzoek van de Faculteit Geneeskunde en Gezondheidswetenschappen van de UGent. Een primair doel was een opmeting van kennis over psychofarmaca van verpleegkundigen, die frequent zorg uitoefenen aan ouderen. Aan de hand van een uitgebreide literatuurzoektocht werden vragen opgesteld, die vervolgens gereviseerd werden door een groep experts (met o.a. artsen, geriaters, gerontologen en farmacologen). Na de uitwerking van evaluatiemethoden, werd een Nederlandstalige kennistest verkregen bestaande uit 24 vragen uit drie thema’s (farmacologische eigenschappen bij ouderen, hypnosedativa en antipsychotica).

Na validatie en betrouwbaarheidstesten werd een kennistest bekomen met een goede stabiliteit, moeilijkheid en degelijke inhoudsvaliditeit. De kennistest kan onderscheid maken tussen personen met hogere of lagere kennis met betrekking tot het onderwerp. De test werd voorgelegd aan 317 verpleegkundigen en studenten verpleegkunde om een eerste opmeting van kennis te maken. De test bracht aan het licht dat 64% van de testpersonen van zichzelf vond over onvoldoende kennis te beschikken. De laag gepercipieerde kennis kan duiden op een onderaanbod rond deze materie in leerstof tijdens hun opleiding. Gemiddeld scoorden beide groepen onvoldoende op de test: verpleegkundigen scoorden gemiddeld 47%, studenten verpleegkunde gemiddeld 38%. Dit actueel kennistekort kan nefast zijn voor de zorg die geboden wordt aan de oudere persoon. Onvoldoende kennis houdt een gedaalde educatie- en signaleringsrol in voor de verpleegkundige. Door de opsplitsing in thema’s kan een gerichte bijscholing of herscholing gerealiseerd worden, teneinde de kennistekorten weg te werken.

Alvorens de kennistest ten volle in werkveld kan benut worden, werd een herformulering van enkele vragen aanbevolen. Sommige vragen werden als moeilijk interpreteerbaar beschouwd, waardoor een vertekend beeld kan opgetekend worden. Pas daarna zal de kennistest transfereerbaar zijn voor verpleegkundigen op niet-geriatrische afdelingen. Wegens de nakende vergrijzing zullen ook deze verpleegkundigen meer patiënten met een geriatrisch profiel dienen te verzorgen. Daarnaast dient ook nagegaan wat de attitude van verpleegkundigen is ten opzichte van psychofarmaca. Bijvoorbeeld kan men zich afvragen of verpleegkundigen voldoende zelfzeker zijn over hun kennis of hun opleiding om met patiënten met gedragsstoornissen om te gaan. Of bieden psychofarmaca een makkelijke uitweg, omdat verpleegkundigen onvoldoende vertrouwd zijn met andere, niet-medicamenteuze behandelingen voor gedragsstoornissen? Vervolgonderzoek zal met andere woorden noodzakelijk zijn om een volledig zicht te krijgen in dit complexe proces.
