Verhandeling aangeboden tot het verkrijgen van de graad van Licentiaat in de Psychologie, juni 2008. Leiding: Prof. Dr. Mia Leijssen – met medewerking van Trui Vercruysse.

 

Deze literatuurstudie tracht een zo volledig mogelijk overzicht te geven van het onderzoek naar de lange termijn gevolgen van het overleven van kinderkanker. De voorbije decennia is zowel de incidentie als de overlevingskans van kinderen met kanker aanzienlijk gestegen, waardoor het absolute aantal overlevenden ook is toegenomen. Hierdoor ontstond interesse voor deze relatief nieuwe problematiek. Dit onderwerp wordt reeds verschillende jaren exhaustief onderzocht. Daarom beperkt deze studie zich tot de meest recente literatuur, zijnde vanaf het jaar 2000.

Het eerste hoofdstuk geeft een beknopte rondleiding in de medische aspecten van kanker bij kinderen. Na een korte introductie over wat kanker nu precies inhoudt, worden de belangrijkste incidentiecijfers en trends weergegeven. Daarna volgt een overzicht van de meest voorkomende vormen van kanker bij kinderen, gevolgd door een schets van de mogelijke behandelingsmethoden. Hier wordt ook kort ingegaan op de lichamelijke bijwerkingen van de ziekte en haar behandeling. Aan de hand van de significant betere overlevingscijfers wordt de overgang gemaakt naar het tweede hoofdstuk.

Dit tweede hoofdstuk gaat dieper in op de psychosociale aspecten van kinderkanker. Er wordt voor zowel het kind, de ouders als de broers en zussen nagegaan wat de ziekte voor hen teweeg heeft gebracht. Het hele ziekteproces wordt onder de loep genomen, beginnende bij de diagnose. Er wordt kort ingegaan op de psychologische impact van de behandeling. Daarna komen de implicaties van het einde van de behandeling aan bod. Er begint voor de patiënt en zijn omgeving een hoopvolle maar onzekere periode. Een belangrijk deel van dit hoofdstuk wordt gewijd aan de lange termijn gevolgen van hun ziekte op vlak van sociale en cognitieve vaardigheden, schools functioneren, internaliserende en externaliserende problemen. Tot slot worden de beperkingen van het onderzoek belicht.

Het derde hoofdstuk biedt een overzicht van nazorginitiatieven voor kinderen die kanker overleefden en hun familie. De aandacht wordt gevestigd op wereldwijde en nationale initiatieven, op Vlaamse organisaties en op projecten op niveau van het ziekenhuis en de individuele patiënt.
