Waar staan we?

Elk  jaar wordt er in Europa 1 kind op 2000 geboren met Spina Bifida. Deze aangeboren aandoening aan het centraal zenuwstelsel kan leiden tot ernstige, levenslange gevolgen. Wereldwijd is deze diagnose voor bijna 30% van de getroffen boorlingen een doodsvonnis.

Om deze gevolgen te vermijden wordt er na +/- 20 weken zwangerschap overgegaan tot een openbuikoperatie. Deze ingreep is zeer invasief en gaat gepaard met een significante kans om de foetus af te stoten. Daarnaast leidt het voor de moeders vaak tot een lang herstelproces met vaak permanente schade aan de buik- en baarmoederwand

Sinds enkele jaren ontwikkelt de medische wereld een minimaal invasieve operatie om Spina Bifida te behandelen. Het principe hiervan is om via één of meerdere kleine openingen een camera en instrumenten in het lichaam te brengen. Met deze innovatieve ingreep wilt men de risico’s voor moeder en kind verminderen en de levenskwaliteit van het kind maximaliseren.

Deze nog steeds evoluerende en complexe werkwijze vergt echter enorm veel training. Het kost naar zeggen meer dan €200 000 om een opgeleid chirurg bij te scholen tot minimaal invasieve chirurg met de nodige competenties om deze ingreep tot een goed eind te brengen. Een groot deel van deze kosten gaan naar proefdieren die gebruikt worden om een realistisch in vivo trainingsscenario te verschaffen.

In deze interdisciplinaire masterproef ontwikkelden we een synthetisch trainingsmodel dat de chirurgen kan helpen om effectiever te trainen voor de Spina Bifida ingreep. Door chirurgen te kunnen tracken terwijl ze oefenen op een herbruikbaar specimen, zorgen we voor een waardevolle en duurzame bijdrage aan hun trainingsprogramma. 

 

Kunststof, sensoren en een snuifje wiskunde

Bij Spina Bifida groeit de wervelkolom niet helemaal samen en is er daardoor een zone op de onderrug die misvormd is. Het resultaat is een delicaat defect dat een zeer preciese behandeling vereist. Het door ons ontwikkeld trainingsmodel bootst deze gedetailleerde structuur na met verschillende laagjes van polymeren. Door het incorporeren van enkele sensoren kunnen we ook meten of de krachten en handelingen van de chirurg de delicate zones van de foetus niet beschadigen. 

Om de kans op afstoting van de vrucht tijdens de operatie te minimaliseren voegden we daarnaast ook een synthetische uteruswand gekoppeld met een numerieke simulatie toe. Hierdoor kunnen chirurgen bij elke beweging weten hoeveel kracht ze uitoefenen op de delicate vruchtzak en bijgevolg hun werkwijze aanpassen.

In de toekomst is het de bedoeling dat dit model deel wordt van het trainingsprogramma voor de minimaal invasieve operatie voor Spina Bifida. Door het gedetailleerder trainen en opvolgen van de chirurg hopen we de kost en het dierenleed dat gepaard gaat met de opleiding te verminderen.

Dit resultaat is een bescheiden schakel in het geheel van deze evolutie in de medische wereld. Desalniettemin is het een stap verder in de richting van een situatie waar we geen kinderen of moeders meer verliezen en is het een kans om als biomedisch ingenieur een bijdrage te leveren aan deze steeds innoverende wereld..
