Post-mortem onderzoek, wanneer de doden de levenden bijstaan

Inleiding

‘Het tijdstip van overlijden is 13u01’: zegt de arts op een vastberaden toon. Het onverwachte piepende geluid van de hartmonitor vult samen met onbegrip de kamer van mevrouw Finley. ‘Wat is er gebeurd?’: klonk het in ieders hoofd. Twee verpleegkundigen voeren systematisch de lijktooi uit en rollen het bed door de lange gangen van het ziekenhuis. De luide bel van het mortuarium kondigt de aankomst van mevrouw Finley aan. Hier staat een vriendelijk gezicht de verpleegkundigen op te wachten in een sterk belichte gang met een eigenaardige geur waar je je vinger maar niet op kan leggen. De arts heeft een klinische autopsie aangevraagd. Dit betekent de start van een nieuw verhaal, weliswaar het laatste verhaal van mevrouw Finley.

Klinische autopsie

Het woord 'autopsie' is afkomstig van het Griekse autopsia: 'met eigen ogen zien'. Of zoals de literatuur het weergeeft; een gestandaardiseerd onderzoek van een overledene voor wetenschappelijke of medisch-juridische doeleinden. De autopsie is onderverdeeld in twee types, de klinische en de forensische autopsie. We kennen allemaal de forensische autopsie van populaire misdaadseries zoals Bones, NCIS, Body of Proof, enz. Maar wat is nu een klinische autopsie? Wat kan het ons vertellen? En hoe kan het een elementaire bijdrage leveren aan niet alleen de nabestaanden, maar de huidige maatschappij?

Een klinische autopsie is een autopsie die wordt uitgevoerd in het ziekenhuis waar de patiënt overleed. De behandelende arts wil meer duidelijkheid over het overlijden of de pathologie en vraagt aan de (klinisch) patholoog verder onderzoek. Ondanks de ziekenhuisopname, betekent dit niet noodzakelijk dat de doodsoorzaak bekend is. Het doel van een klinische autopsie beperkt zich niet alleen tot het identificeren van de oorzaak en wijze van overlijden. Het is ook een bevestiging en herziening van de ziektetoestand van de patiënt; was de diagnose juist? Was de kanker verder uitgezaaid? Heeft de geboden therapie het juiste effect gehad? Was er een andere ziekte die we gemist hebben? Kortom, de vastgestelde ziekten worden een laatste maal getoetst met de diagnose(n) en de gegeven behandeling(en). Dit wordt ook de clinico-pathologische correlatie genoemd. Het belang van post-mortem onderzoek wordt in het juiste perspectief geplaatst wanneer op wetenschappelijk vlak (grote) discrepanties (d.i. tegenstijdigheden) tussen de resultaten van de autopsie en de diagnose(n) bij leven worden aangetoond. Naast de wetenschappelijke bijdrage primeert evenzeer de bijdrage voor de nabestaanden; een laatste éénduidig antwoord met het opperste respect voor de overledene.

Wereldwijde daling van autopsies

De autopsie heeft in de loop van de geschiedenis een uitzonderlijke bijdrage geleverd in de kennis van de anatomie en ziekten. Het was als het ware de gouden standaard in de geneeskunde. Met de komst van de technologische (r)evolutie kwam daar echter verandering in…

Er is op enkele decennia tijd een internationale dramatische daling van het aantal autopsies van 1 op 3 overlijdens in de jaren 90 naar het actuele 2 op 100. Vooral de geloofsovertuiging, de tijdsgeest van onschendbaarheid en zelfbeschikking, het blinde vertrouwen in moderne technologie en de lage financiële compensaties zijn bij de belangrijkste factoren voor deze mondiale keldering. Toch heeft een daling van post-mortem activiteiten rechtstreeks betrekking op zowel sociaal-maatschappelijk (geen antwoorden op doodsoorzaak), gezondheidszorgeconomie (foutieve gezondheidsstatistieken) als de medische technologie en curriculum (geen toetsing van nieuwe technologie/therapie, desinteresse van jonge artsen en daling van autopsiekennis).

Meer dan cijfers

Onze studie evalueerde de autopsieaanvragen van 2015 tot en met 2017 in het Universitair Ziekenhuis Leuven (UZ Leuven). UZ Leuven is een tertiair centrum wat betekent dat de moeilijkste en zwaarste aandoeningen hier worden behandeld. Van de 555 autopsieaanvragen voldeden 282 gevallen aan de vooropgestelde criteria. Het klinische aanvraagformulier dat zowel de klinische informatie als doodshypothese bevat, werd vergeleken met de conclusie van het autopsieverslag. In 88,8% van de gevallen beantwoorde de vraag van de aanvragende arts aan de conclusie van de patholoog. In 21.4% van de gevallen werd de klinische diagnose weerlegt en een nieuwe doodszoorzakelijke ziekte gevonden na autopsie. Met andere woorden, de autopsie heeft in één op vijf gevallen nieuwe bevindingen naar boven gebracht. Is dit veel?

Moeten we ons zorgen maken? Hoe staan we ten opzichte van de rest van de wereld? In de literatuur zien we dat het gros van de studies een medische beoordeling uitvoerde waarbij de focus wordt gelegd op het hele medische dossier (opname, hospitalisatie, diagnose, behandeling, enz.). Onze studie is gericht op een kwalitatieve beoordeling van de autopsie-aanvraag en de autopsieconclusie. Verrassend genoeg zien we opvallende gelijkenissen. Niet alleen wereldwijd, maar ook in UZ Leuven zelf. Hier onthulde een vroegere studie door Roosen et al. in 2000 een discrepantiepercentage van 26%. Deze bevinding is opmerkelijk aangezien de discrepanties vergelijkbaar zijn, hoewel er bijna twee decennia verstreken zijn, inclusief verbeteringen in diagnostische technieken. Uit de literatuur blijkt dus dat ondanks de enorme technologische vooruitgang de foutmarge sinds 1912 niet verbeterd is maar gelijk is gebleven. Kortom, de technologische (r)evolutie heeft er niet voor gezorgd dat we bij de autopsie minder nieuwe of andere ziekten vinden. Erg beangstigend als je denkt dat het aantal autopsies wereldwijd aan een vrije val bezig is.

Conclusie

De wereldwijde vrije val van het aantal autopsies doet denken dat de meeste artsen ervan uitgaan dat de technologische (r)evolutie een betere klinische diagnostiek mogelijk heeft gemaakt, in die mate dat post-mortem onderzoek niet meer moet aangevraagd worden. Toch toont dit onderzoek, evenals de internationale literatuur, aan dat het aantal gemiste doodsoorzakelijke ziektes niet is afgenomen maar gestabiliseerd. We hopen dat het bewustzijn, de noodzaak en de bijdrage met betrekking tot klinische autopsies zullen toenemen. Aangezien de kwaliteit van de gezondheidszorg niet alleen afhankelijk is van de diagnostische en therapeutische methoden, maar ook van de beoordeling van hun effectiviteit en de exacte doodsoorzaken door middel van post-mortem onderzoeken.

In het oudste anatomische theater (1594), gebouwd voor de universiteit van Padua te Italië, staat nog steeds het volgende gegraveerd in Latijn:“Hic est locus ubi mors gaudet succurrere vitae”. In het Nederlands: “Dit is de plaats waar de dood zich verheugt om de levenden te helpen”. Een spreuk die desondanks nog even actueel is.
