Vooronderzoek naar de optimale neurofysiologische parameters, eigen aan spierstimulatie, bij personen zonder neuromusculaire aandoeningen.

Inleiding

Dit vooronderzoek werd uitgevoerd voorafgaandelijk aan de eigenlijke studie. De eigenlijke studie wil het effect nagaan van elektrostimulatie als behandeling van slikstoornissen bij patiënten met een hersenbloeding. Door het uitvoeren van een literatuurstudie kwamen er al snel wat probleempunten naar boven waardoor dit vooronderzoek noodzakelijk werd. Elektrostimulatie is een therapievorm waarbij op onwillekeurige basis, door middel van een elektrisch toestel, een elektrische stroom wordt opgewekt om een spier te laten samentrekken. Dit is een techniek om spieren te versterken die verzwakt zijn.

Hoofdtekst

Het vooronderzoek werd uitgevoerd bij gezonde personen. Dit wil zeggen dat de deelnemers niet mogen lijden aan een acute en of chronische aandoening, noch slikproblemen mogen vertonen.

In dit onderzoek werden 3 grote doelen opgenomen:

·         Het echografisch in beeld brengen van de slikspieren en hierop metingen uitvoeren

·         Het bepalen van de dieptewerking van twee verschillende elektrostimulatievormen

·         De invloed van de  plaatsing van de elektroden op de slikspieren nagaan

We zijn bij het schrijven van deze scriptie vertrokken vanuit een wetenschappelijk artikel van German et al. (2011) over het belang van de beweging en de functie van het tongbeen tijdens het slikken. Al vlug bleek dat de overgrote meerderheid van de wetenschappelijke artikels gebaseerd zijn op de eigenschappen van arm- en beenspieren. Hierbij werd er geen rekening gehouden met het feit dat de samenstelling van de slikspieren grotendeels verschilt van de arm- en beenspieren. Hieruit volgt dat zowel de biomechanische alsook de neurofysiologische eigenschappen van de desbetreffende spieren nog steeds onbekend terrein is.

Binnen het eerste onderzoeksdoel werden er metingen uitgevoerd op de slikspieren met behulp van echografie. De meerwaarde van dit meetinstrument is onder andere het exact kunnen lokaliseren van de spieren, het bepalen van de individuele diepteligging, alsook het kunnen aantonen van eventuele spierverzwakking.

Binnen het tweede onderzoeksdoel werd door middel van echografie de dieptewerking van de verschillende stroomvormen bepaald.

Tot slot werd binnen het laatste onderzoeksdoel door middel van echografie de invloed van de plaatsing van de elektroden op de desbetreffende spieren onderzocht. Waaruit we kunnen concluderen dat de plaatsing van de elektroden van cruciaal belang is bij het toepassen van elektrostimulatie.

Conclusie

Echografie is een gemakkelijk te hanteren en een veilig toepasbare methode om bepaalde  wetenschappelijke onderzoeken te verrichten zowel op proefpersonen als op patiënten. Door gebruik te maken van deze techniek waren we in de mogelijkheid om bepaalde twistpunten op een objectieve wetenschappelijke basis te weerleggen en te bewijzen.

Slikproblemen komen frequenter voor. Niet alleen ten gevolge van ziektebeelden zoals hersenbloeding, ziekte van parkinson, multiple sclerose,… maar ook tijdens het verouderen treden er slikproblemen op. Daarom is het noodzakelijk om een degelijk beeld te krijgen over hoe deze slikproblemen ontstaan en welke therapievormen het meest effectief zijn afhankelijk van de te behandelen ziekten.
