Ontwikkeling van numerieke methoden voor thermotherapie van leverkanker

Radiofrequente ablatie (RFA), een vorm van warmtetherapie, is een innovatieve techniek bij de behandeling van leverkanker. Frederik Soetaert ontwikkelde voor zijn masterproef in de ingenieurswetenschappen een computerprogramma om RFA te personaliseren en te optimaliseren. Op termijn kan dit leiden tot een betere prognose voor patiënten met leverkanker.

Klassieke aanpak ontoereikend

Leverkanker treft jaarlijks 740.000 mensen, wat het de vijfde meest voorkomende kanker ter wereld maakt. De klassieke behandeling van leverkanker bestaat uit een combinatie van chirurgie en chemotherapie. Sommige patiënten verdragen de nevenwerkingen van deze behandelingen echter niet of hun algemene toestand laat zware chirurgische ingrepen niet meer toe. Hierdoor heeft iemand met leverkanker slechts 12% kans om na vijf jaar nog in leven te zijn. Een alternatieve aanpak zoals thermotherapie is veelbelovend.

Warmte als bondgenoot in de strijd tegen leverkanker

Thermotherapie bij kanker is een verzamelnaam voor behandelingen die tumoren bestrijden met lokale warmte. In zijn scriptie bestudeerde Frederik Soetaert thermotherapie, onder de vorm van RFA, bij de behandeling van leverkanker. Bij RFA brengt de arts een naaldelektrode in de levertumor. Vervolgens wordt via de elektrode een alternerende elektrische stroom geïnjecteerd. De elektrisch geladen deeltjes aanwezig in het leverweefsel trachten de veranderende stroom te volgen. Hierdoor bewegen ze rondom hun evenwichtspositie en ontstaat er warmte. Deze warmte zorgt voor een temperatuurstijging in de onmiddellijke nabijheid van de naaldtip. Dit geeft aanleiding tot sterke biologische effecten en resulteert uiteindelijk in het afsterven van de tumorcellen.

Radiofrequente ablatie heeft heel wat voordelen ten opzichte van de klassieke behandelmethoden. In vergelijking met chirurgie is het slechts minimaal invasief. Er is maar een kleine snede nodig om de elektrode in te brengen in de lever. In tegenstelling tot chemotherapie is er enkel een lokaal effect. De schade blijft beperkt tot de regio rond de elektrode, de ablatiezone. Het gezonde weefsel blijft hierbij gespaard.

Computers als hulpmiddel in de strijd tegen leverkanker

Artsen passen al enkele jaren RFA toe. Helaas kunnen ze onvoldoende nauwkeurig voorspellen wat de uitkomst van de behandeling zal zijn. Hier is een cruciale rol weggelegd voor ingenieurs. Om een beter inzicht te krijgen in de effecten van RFA ontwikkelde Frederik Soetaert een simulatieprogramma. Dit programma is in staat om patiëntspecifieke MRI-beelden van de lever in te lezen en te voorspellen wat de uitkomst van RFA zal zijn.

Het ontwikkelen van een simulatieprogramma vereist het omzetten van complexe biologische processen in wiskundige vergelijkingen. Dit wiskundig model dient de biologische werkelijkheid zo goed mogelijk te beschrijven. Vooreerst moet het model op elke plaats de temperatuur veroorzaakt door het aanleggen van de elektrische stroom kunnen berekenen. Vervolgens dient het ook in staat te zijn het uitdijen van de warmte doorheen de lever te beschrijven. Dit is een allesbehalve  eenvoudige opdracht aangezien er in biologisch weefsel overal bloedvaten aanwezig zijn die warmte  aan- of afvoeren. Tenslotte moet het model kunnen beslissen wanneer een behandelde cel afgestorven is.

Simulatieresultaten leiden tot verbeteringen

Met behulp van het ontwikkelde simulatieprogramma kunnen we de therapie testen op modeltumoren. Het blijkt dat klassieke RFA wel degelijk in staat is om cellen te doden. Helaas gebeurt dit enkel in een kleine sferische regio van enkele millimeters rondom de naaldtip.Het onderzoek van Frederik Soetaert spitste zich toe op het vinden van betere procedures.

Dit onderzoek toont vooreerst aan dat een bipolaire configuratie bij RFA-behandeling van levertumoren leidt tot betere resultaten. In deze bipolaire opstelling gebruikt men twee naalden (in plaats van één naald in de klassieke monopolaire procedure). De eerste naald injecteert een bepaalde alternerende elektrische stroom, terwijl de tweede naald eenzelfde stroom extraheert. Hierdoor ontstaat gelijktijdig rondom elke naaldtip een ablatiezone. Daarenboven kan de ene ablatiezone de andere beïnvloeden en kan er schade aangericht worden in een groter gebied.Een tweede verbetering bestaat in het aanleggen van gepulste stromen waardoor de wederzijdse invloed van de ablatiezones groter wordt. Bij een gepulste stroom wordt de stroom tijdelijk onderbroken. De periode waarin er geen stroom is, noemen we de afschakeltijd. Hoe langer de afschakeltijd, hoe meer de opgewekte warmte uitdijt en dus hoe groter de ablatiezone wordt. Tijdens deze afschakeltijd daalt de absolute temperatuur echter in de behandelde zone. Bijgevolg vermindert ook de aangerichte schade. In deze tegengestelde effecten zit de grootste innovatie van het nieuwe computermodel. Voor elke patiënt gaat het programma op zoek naar de optimale waarde voor de afschakeltijd. Het programma bepaalt de afschakeltijd die enerzijds toch de minimale temperatuur oplevert die de kankercellen doodt en anderzijds zorgt voor een maximale ablatiezone. In plaats van twee afzonderlijke sferische ablatiezones rondom elke naaldtip, ontstaat er met deze optimale afschakeltijd één grote cilindrische ablatiezone. Dit laat alvast het beste verhopen voor de behandeling van grotere levertumoren.

Toekomstmuziek

Tijdens deze masterproef ontstonden de contouren van de nieuwe bipolaire configuratie voor radiofrequente ablatie. Simulaties tonen aan dat deze configuratie in staat is om voor elke afzonderlijke patiënt een verbeterde behandeling aan te bieden en de levertumoren zo goed mogelijk te verschroeien. In het kader van een doctoraat zal Frederik Soetaert deze technieken verder onderzoeken. Naast het optimaliseren van de elektrische stromen zal bijkomend gebruik gemaakt worden van magnetische velden in de strijd tegen leverkanker.
