‘Healthy lives for all’: een zorg voor allen of een zorg voor enkelen?

Inleiding

Het recht op gezondheidszorg werd in 1948 ingeschreven in de Universele Verklaring van de Rechten van de Mens. Desondanks zijn er vandaag nog grote verschillen op te tekenen tussen de toegang en de kwaliteit van de gezondheidszorg in Westerse landen en ontwikkelingslanden. Een kind geboren in Sub-Sahara Afrika heeft nog steeds 14 keer meer kans om te overlijden voor zijn vijfde verjaardag dan een kind geboren in het Westen.  

Universal Health Coverage: sociale bescherming binnen de gezondheidszorg

In deze masterthesis wordt een blik geworpen op het antwoord dat de internationale gemeenschap de afgelopen decennia heeft geformuleerd op deze ongelijkheid binnen de gezondheidszorg. Verschillende strategiën werden gelanceerd waarvan de Millenium Development Goals (MDGs) en de Sustainable Development Goals SDGs) de meest recente zijn. Toen in 2015 de balans werd opgemaakt van 15 jaar ontwikkelingshulp binnen het kader van de MDGs, bleek dat geen enkele van de drie doelstellingen binnen het domein van gezondsheidszorg volledig behaald was. Hoe kan dit verklaard worden? Binnen dit werk wordt een verband gelegd tussen het falen van de MDGs en de afwezigheid van sociale beschermingsmechanismen. Onze eigen welvaartstaat heeft in het verleden reeds bewezen dat er geen vooruitgang kan geboekt worden in het bestrijden van armoede en ongelijkheid wanneer de zwakste in onze samenleving niet beschermd worden. De SDGs, het nieuwe internationale ontwikkenlingsplan van de Verenigde Naties, lijkt deze visie te ondertsteunen en implementeerde het concept van Universal Health Coverage (UHC) binnen één van zijn doelstellingen. Binnen UHC wordt het voorzien van een universele ziekteverzekering gezien als de sleutel tot een kwalitatieve, rechtvaardige en universeel toegankelijke gezondheidszorg. Op deze manier wordt er gestreefd naar ‘healthy lives for all’ tegen 2030.

Universal Health Coverage in Tanzania

Maar hoe realistisch is het installeren van ziekteverzekering voor elke burger op onze planeet? Om even te duiden, Duitsland heeft er 127 jaar over gedaan om zijn ganse bevolking te voorzien van een ziekteverzekering. Kunnen we werkelijk van ontwikkelingslanden verwachten dat ze dit binnen 15 jaar kunnen realiseren? Om een antwoord te formuleren op deze vraag werd in deze studie de case van Tanzania gebruikt. Met een gemiddeld jaarijks inkomen van $950 en een 151ste plaats op de Human Development Index, behoort het Oost-Afrikaanse land tot één van de minst ontwikkelde landen in de wereld. Wanneer het gezondheidsfinancieringssyteem van Tanzania onder de loep werd genomen,  blijkt  dat Tanzania reeds sinds de beginjaren 2000 aan het investeren is in UHC. Een belangrijke stap werd genomen door het oprichten van het Community Health Fund (CHF) in 2001. Het CHF was een ziekteverzekering voor de informele sector, mensen niet tewerkgesteld door een arbeidscontract,  in Tanzania en had tot doel om (gratis) toegang tot basis gezondheidszorgen te voorzien in landelijke gebieden. Desondanks de goede intenties is het CHF er echter nooit in geslaagd om deze informele sector in landelijke gebieden voldoende te bereiken. In 2016 was amper 15.5 procent van de Tanzaniaanse bevolking aangesloten onder het CHF.  

Het alternatief van de Public-Private Partnerships

De case van Tanzania toont aan dat het ondersteunen van de UHC-visie niet voldoende is om een systeem te organiseren dat elke burger voorziet van een ziekteverzekering. Het tweede deel van dit onderzoek focust zich dan ook op alternatieve initiativen die tot doel hebben om een bijdrage te leveren aan UHC. Het alternatief belicht doorheen deze studie is het concept van public-private partnerships (PPP). Een PPP is een samenwerkingsverband tussen de overheid en een private partner om een gemeenschappelijk doel te bereiken waarbij middelen en risico’s worden gedeeld. Zo besloot de Tanzaniaanse overheid om in zee te gaan met PharmAccess, een Nederlandse NGO, om het CHF te herstructureren. Deze samenwerking leidde tot de oprichting van het improved CHF (iCHF) in 2014. Om te onderzoeken of het iCHF werkelijk een meerwaarde betekende voor UHC werden er 372 vragenlijst afgenomen bij iCHF-members in de Kilimanjaro-regio in Tanzania. De verzamelde data werd vervolgens gestructureerd volgens een analytisch kader gebaseerd op de UHC-cube. Deze cube, ontwikkeld door de Wereldgezondheidsorganisatie, stelt dat er naar UHC gewerkt wordt door te investeren in het aandeel van de bevolking dat gedekt is, het aantal diensten en de directe kosten die gedekt worden.

Een zorg voor allen

In mei 2017, drie jaar na de opstart van iCHF, meldde PharmAccess dat er zich reeds 212.145 mensen hadden aangesloten bij het programma. Deze cijfers zeggen echter niet zoveel als we geen analyse kunnen maken in het licht van UHC. Deze UHC-analyse werd in deze studie dus wel uitgevoerd voor de 71.559 iCHF-leden in de Kilimanjaro-regio. Eén van de belangrijkste resultaten was dat iCHF er wel degelijk in slaagt om mensen met een laag inkomen in landelijke gebieden te bereiken. De resultaten wezen echter ook uit dat de kwantiteit en de kwaliteit van de geleverde diensten nog al te vaak te wensen overlaat. Het hebben van een verzekering is dus geen garantie op een goede gezondheidszorg, maar moet deel uitmaken van een groter geheel. Een geheel waarbij niet alleen wordt gekeken naar de verantwoordelijkheden en inspanningen van lokale of nationale overheden, maar waar ook private partners binnen én buiten de zorgsector, non-profit én profit en de gemeenschappen zelf hun plaats in hebben. Een geheel waarbij niet enkel gefocust wordt op financiële bescherming voor de allerarmsten, maar waarbij ook billijkheid en kwaliteit in rekening wordt gebracht. Alleen dan kan het recht op gezondheidszorg werkelijk gevrijwaard worden en kunnen internationale ontwikkelingsplannen hun ambitieuze doelstellingen voor een gezonde wereldbevolking verwezenlijken.
