Leidt blootstelling aan straling tot vroegtijdige veroudering van hart en bloedvaten?

We horen het elke dag: om ons risico op hart- en vaatziekten te verlagen moeten we gezond eten, regelmatig sporten en mogen we niet roken. De bezorgdheid over de toename in het aantal hart- en vaatziekten is echter terecht want het is wereldwijd de meest voorkomende doodsoorzaak. Recent onderzoek heeft aangetoond dat ook blootstelling aan ioniserende straling, bijvoorbeeld door het toepassen van medische beeldvorming of door nucleaire rampen, het risico op hart- en vaatziekten kan verhogen. Ioniserende straling is straling die voldoende energie bezit om schade te kunnen toebrengen aan ons lichaam. Onder andere door het vroegtijdig laten verouderen van onze bloedvatcellen kan stralingsblootstelling leiden tot een verhoogd risico op hart- en vaatziekten. De studie van vroegtijdige veroudering van endotheelcellen, het type bloedvatcellen die de binnenkant van ons hart en onze bloedvaten aflijnen, na blootstelling aan straling was het onderwerp van mijn masterthesis.

Hart- en vaatziekten: een sluipend gevaar

Volgens cijfers van de Wereldgezondheidsorganisatie veroorzaken hart- en vaataandoeningen wereldwijd ongeveer 17 miljoen doden per jaar. Elke ziekte die het hart of de bloedvaten treft (bv. een hartinfarct of een beroerte), valt onder de noemer ‘hart- en vaatziekten’. Vaak worden deze ziektes veroorzaakt door aderverkalking. Aderverkalking is het verharden en vernauwen van de bloedvatwand waardoor het de goede werking van onze bloedvaten en ons hart belemmert. Meestal geeft aderverkalking geen symptomen tot het te laat is en het bloedvat al bijna volledig verstopt is. Endotheelcellen zijn essentieel voor een gezond hart en gezonde bloedvaten.  Ze zijn belangrijk voor de vorming van nieuwe bloedvaten, spelen een rol bij het reguleren van de bloeddruk en bloedstolling en hebben een ontstekingswerende functie. Wanneer deze cellen niet meer goed werken, kan dit leiden tot aderverkalking. Een te hoge cholesterolspiegel, roken, hoge bloeddruk en diabetes, maar ook blootstelling aan straling zijn factoren die ervoor kunnen zorgen dat het endotheel beschadigd wordt en niet meer naar behoren functioneert.

Straling doet onze bloedvatcellen vroegtijdig verouderen

Eerder onderzoek heeft aangetoond dat blootstelling van bloedvatcellen, waaronder endotheelcellen, aan straling ervoor zorgt dat deze cellen vroegtijdig gaan verouderen oftewel ‘senescent’ worden. Senescente cellen leven nog wel maar ze delen niet meer en zijn niet meer in staat hun normale functie uit te oefenen. Ook wanneer cellen ouder worden en hun maximale delingscapaciteit bereikt hebben, worden ze senescent. Na blootstelling aan straling gebeurt dit echter veel vroeger dan normaal, vandaar de naam vroegtijdige veroudering of premature senescentie. Hoe het komt dat endotheelcellen senescent worden na bestraling is nog niet volledig duidelijk. Wel is geweten dat straling schade toebrengt aan het erfelijk materiaal van de cel, het DNA, en dat dit één van de oorzaken is die ervoor zorgt dat de bestraalde cel senescent wordt. Senescente endotheelcellen kunnen niet meer normaal functioneren, wat kan leiden tot aderverkalking. De resultaten van deze thesis toonden aan dat bestraling van humane endotheelcellen met een stralingsdosis, die regelmatig gebruikt wordt voor het bestrijden van tumoren, ertoe leidt dat de endotheelcellen senescent worden na ongeveer 7 dagen na de bestraling. Hierdoor zullen personen met tumoren in de buurt van het hart, bijvoorbeeld bij borstkanker, een verhoogd risico hebben op het ontwikkelen van hart- en vaatziekten na behandeling met radiotherapie.

Hoe kunnen we ons beschermen tegen hart- en vaatziekten veroorzaakt door straling?

Het gebruik van straling voor medische doeleinden is de laatste decennia sterk toegenomen, denk maar aan CT-scans, röntgenfoto’s en radiotherapie. Vooral radiotherapie voor de behandeling van kanker is niet meer weg te denken uit het hedendaagse leven: ongeveer 50% van de kankerpatiënten wordt behandeld met radiotherapie. Hierdoor stijgt enerzijds de overlevingskans van de patiënt maar anderzijds neemt op latere leeftijd de kans op complicaties toe door de stralingsblootstelling. Daarom is het belangrijk om onderzoek te doen naar de langetermijneffecten van straling en naar beschermende maatregelen. Voornamelijk nieuwe soorten radiotherapie en geneesmiddelen die beschermen tegen deze complicaties, zogenaamde radioprotectieve geneesmiddelen, worden momenteel onderzocht.

Er is al veel onderzoek gedaan naar radioprotectieve geneesmiddelen die ons zouden kunnen beschermen tegen de langetermijneffecten van stralingsblootstelling, zoals hart- en vaatziekten. Mijn thesis focuste specifiek op het uittesten van verschillende geneesmiddelen en supplementen die zouden kunnen beschermen tegen vroegtijdige veroudering van endotheelcellen na stralingsblootstelling. Het voorkomen van deze vroegtijdige veroudering zou één van de mogelijke oorzaken van het ontstaan van hart- en vaatziekten na bestraling kunnen wegnemen. Van de vijf onderzochte geneesmiddelen en supplementen kwamen vitamine C en atorvastatine, een cholesterolverlagend geneesmiddel, naar voren als goede kandidaten. De resultaten van ons onderzoek toonden aan dat beiden een effect hebben op de vroegtijdige veroudering van bestraalde endotheelcellen. Er is echter nog meer onderzoek nodig om na te gaan of ze effectief vroegtijdige veroudering van endotheelcellen na bestraling kunnen voorkomen. Daarnaast is het ook zeer belangrijk om andere oorzaken voor het ontstaan van hart- en vaatziekten na bestraling te identificeren. Enkel wanneer alle oorzaken tegelijk aangepakt worden is het mogelijk om aderverkalking en bijgevolg het ontstaan van hart- en vaatziekten na bestraling te voorkomen.

Radioprotectie: belangrijk voor de toekomst

Medische toepassingen die gebruik maken van straling, zoals radiotherapie, zijn onmisbaar geworden in het hedendaagse leven. Wetenschappelijk onderzoek heeft echter aangetoond dat stralingsblootstelling op lange termijn verschillende neveneffecten kan hebben, bijvoorbeeld de ontwikkeling van hart- en vaatziekten. Daarom is het belangrijk om onderzoek te doen naar deze neveneffecten en mogelijke beschermende maatregelen om ze te kunnen vermijden. Momenteel worden er nieuwe soorten radiotherapie ontwikkeld waarbij tumoren preciezer bestraald kunnen worden en er minder gezond weefsel wordt blootgesteld aan straling. Het is echter op dit moment nog niet mogelijk om gezond weefsel volledig te vrijwaren. Onderzoek naar radioprotectieve geneesmiddelen is dus van groot maatschappelijk belang aangezien het de algemene gezondheid en levenskwaliteit na radiotherapie kan verbeteren. Geneesmiddelen die specifiek kunnen vermijden dat endotheelcellen senescent worden na bestraling, zouden deel uitmaken van een reeks radioprotectieve maatregelen die toegepast kunnen worden bij radiotherapie.
