Angst en dentofobie: zowel bij patiënt als practicus?

Misschien zijn angstige tandartsen wel de beste

Bent u ooit naar de tandarts geweest? Hopelijk wel. Misschien hebt u dan wel gemerkt dat dit niet altijd even prettig is. Veel patiënten vertonen angst voor tandheelkundige behandelingen met alle gevolgen van dien: Ze vermijden de tandarts waardoor hun mondgezondheid al maar slechter wordt, patiënten komen dan enkel naar de tandarts wanneer de pijn niet meer te houden is. De gevolgen laten zich raden: volledige extractie, invasieve behandelingen, prothesen, een dure tandartsrekening, nog meer angst, ... Bij sommige patiënten kan deze angst fobische proporties aannemen, dan spreken we van dentofobie of odontofobie. Maar hoe werkt dit nu precies? En hoe ervaren tandartsen het zelf wanneer ze tandheelkundige behandelingen ondergaan?

Mijn scriptie handelt over angst en dentofobie bij patiënten en bij behandelaars. In het eerste deel wordt onderzocht hoe angst bij patiënten tot stand kan komen, hoe frequent het voorkomt, waarom dit een chronisch proces is en wat er aan te doen is. Hier is heel veel onderzoek over gevoerd, met name door professor A. De Jongh die werkzaam is aan de ACTA te Amsterdam. In het tweede deel van de scriptie wordt gekeken hoe tandartsen zelf tandheelkundige behandelingen ervaren. Tot op heden is dit een blinde vlek in het tandheelkundig onderzoek. Wat ervaart u tandarts als men een boor in zijn tanden plaatst, hoe schat hij zijn eigen mondgezondheid in en hoe percipiëren tandartsen angst bij patiënten? Tandheelkundige verenigingen zoals het Verbond der Vlaamse Tandartsen, het Ivoren Kruis en de Nederlandse Maatschappij tot bevordering der Tandheelkunde konden hier ook geen antwoord op geven. Om te onderzoeken hoe tandartsen angst bij patiënten inschatten en hoe ze zelf behandelingen ervaren werd een enquête gehouden bij 1733 Vlaamse tandartsen. Voorafgaande aan de enquête bij tandartsen werd er een pilot studie uitgevoerd bij 77 tandheelkunde studenten. Er werd geopteerd voor een enquête aangezien er geen voorgaande onderzoeken waren om de resultaten mee te correleren en omdat zowel tijd als middelen gelimiteerd waren.

Zoals wel vermoed kan worden gaan zowel tandartsen als tandheelkunde studenten ervan uit een goede tot zeer goede mondgezondheid te hebben. Tandartsstudenten en tandartsen zijn vermoedelijk meer dan andere personen bezig met hun eigen mondgezondheid. De ondervraagde personen lijken tevreden over hun mondgezonheid, desalniettemin laat ongeveer een vijde van de ondervraagde studenten en een derde van de ondervraagde tandartsen zijn eigen mondgezondheid minder dan éénmaal per jaar controleren. Eigen adviezen worden dus niet altijd opgevolgd. Ongeveer 70% van de tandartsen en studenten ervaart nooit terughoudendheid voor het ondergaan van tandheelkundige behandelingen. Ongeveer 4% van de tandartsen en 3% van de studenten ervaart vaak terughoudendheid voor het ondergaan van behandelingen.

Enkling et al. (2006) vroegen in een onderzoek bij 300 voetgangers in een Duitse stad wat een tandarts bij angstige patiënten zou moeten doen om de angst te reduceren. Een meerderheid (69%) wenste meer accurate informatie omtrent de behandeling die ze ondergingen, gevolgd door een empatische tandarts (62%) en een pijnloze behandeling (62%). Uit onze enquête bleek dat studenten en tandartsen, die over de nodige achtergrondinformatie beschikken, ook nog occasioneel terughoudenheid voor de behandelingen ervaren. Naar de kapper gaan zal waarschijnlijk wel altijd leuker blijven dan naar de tandarts gaan.

Wanneer gekeken wordt of studenten en tandartsen nadenken over complicaties die kunnen optreden tijdens behandelingen kan geconcludeerd worden dat dit zeker een rol speelt bij de perceptie van de behandeling door zowel studenten als tandartsen. Voor de hoeveelheid angst te meten werd gewerkt met een VAS-score tussen nul en tien. Een score van nul gaf een laag angstniveau weer, een score van tien een hoog angstniveau. Ook voor de gedachten omtrent complicaties werd gewerkt met een gelijkaardige VAS-score. Een score van nul gaf weer dat tandartsen en studenten niet over de complicaties nadachten, een score van tien gaf weer dat men altijd over de complicaties nadacht. VAS-scores voor angst waren over het algemeen laag. VAS-scores hoger dan zeven werden zowel door studenten als tandartsen vaker toegekend voor cognities omtrent complicaties dan voor angst. Prevalentiewaardes omtrent angst zijn sterk gegroepeerd rond de laagste VAS-scores terwijl de VAS-scores omtrent de gedachten aan complicaties een meer verspreid beeld vertonen.

Wanneer we vroegen aan de tandartsen waar ze angst voor hadden tijdens een behandeling bleek dat men meestal nergens angst voor had, hierop volgde de verdoving en een vijfde van de tandartsen zou angst ervaren bij een extractie.

Deze samenvatting is slechts een zeer summiere en gefragmenteerde weergave van mijn scriptie. Toch kunnen we concluderen dat tandartsen ook maar mensen zijn en evenals patiënten niet altijd even erg uitkijken naar een behandeling bij de tandarts. Belangrijk is dat patiënten beseffen dat tandartsen proberen behandelingen zo aangenaam mogelijk te maken. Wie zou u het liefst als tandarts hebben: Iemand die zelf begrijpt dat tandheelkundige behandelingen niet leuk zijn en mogelijk kunnen leiden tot complicaties of een tandarts die beweert zelf nooit angstig te zijn en niet nadenkt over complicaties? Misschien zijn ‘angstige’ tandartsen wel de beste.
