Spraak- en mondgewoonten bij patiënten met verschillende types van implantaten in de onderkaak met een 'All-on-Four' concept.

Vervangt 'All-on-Four' binnenkort het klassieke kunstgebit?

Voor de jeugd is het nog een ver-van-mijn-bedshow, maar steeds meer ouderen uiten hun ontevredenheid over hun klassieke gebitsprothese, beter gekend als het kunstgebit. Vaak belandt die prothese dan ook op de kast en gaan mensen tandeloos door het leven. Een handig alternatief is dan implantatie volgens de 'All-on-Four' procedure, waarbij de prothese blijvend verankerd wordt op vier implantaten. De Universiteit Gent onderzocht recent de invloeden op de spraak- en mondgewoonten van implantatie in de onderkaak door middel van de 'All-on-Four' procedure. De klassieke klachten zoals 'pijn bij het kauwen', 'problemen met spreken en eten' en ook heel wat psychosociale moeilijkheden die gepaard gaan met de klassieke gebitsprothese, lijken hiermee voorgoed verleden tijd. Voor velen lijkt het een droom, voor enkelen is het reeds  werkelijkheid.

Tandeloos door het leven gaan wordt niemand toegewenst. De oplossing die vroeger het meest werd geboden, was een klassieke uitneembare gebitsprothese. Aanvankelijk zijn patiënten er enorm mee gebaat. Pas na enige tijd treden bij heel wat personen de eerste en vaak gelijkaardige problemen op: de tanden komen losser te zitten, er ontstaan problemen bij het spreken en het sociaal contact vermindert uit angst om de gebitsprothese tijdens een gesprek te verliezen.

Een twintigtal jaren geleden werd er onderzoek gevoerd naar een techniek die in de tandheelkunde haar toepassing nog niet had gevonden, namelijk implanteren. Hierbij wordt de patiënt voorzien van één of meerdere kunstwortels waarop een vaste gebitsprothese geplaatst kan worden. Dit met als voornaamste doelstellingen het herstel van de kauwfunctie en de esthetiek. Dankzij implantatie hebben patiënten de angst niet meer om de prothese te verliezen tijdens het kauwen of het spreken.

Toch staat ook in dit domein de wetenschap niet stil en wordt er nog steeds onderzoek verricht ter optimalisatie van de reeds gekende technieken. In opdracht van de Universiteit Gent voerde Sofie De Roo, studente logopedie, onderzoek naar de invloed op de spraak- en mondgewoonten van de 'All-on-Four' techniek in de onderkaak. Hiervoor werd nauw samengewerkt met de dienst Parodontologie van het UZ Gent. In samenspraak met de behandelende parodontologen werd een proefgroep van negen patiënten samengesteld, die zowel voor als na de ingreep verschillende spraaktests onderging. Op die manier kon De Roo een vergelijkende studie uitvoeren tussen de spraak- en mondgewoonten voor en na de ingreep.

Het ‘All-on-Four’ concept

‘All-on-Four’ is een term die nog niet gekend is bij het grote publiek. Zoals die doet vermoeden, worden patiënten voorzien van vier implantaten die voor dit onderzoek in de onderkaak werden geplaatst. Daarop wordt nadien een vaste prothese  geplaatst. Uniek bij dit concept is dat de prothese al twee dagen na de ingreep op de implantaten geplaatst kan worden, met een snel herstel van de kauwfunctie en esthetiek als positief gevolg.

Invloeden van het 'All-on-Four' concept op het vlak van spraak- en mondgewoonten konden in dit onderzoek niet worden vastgesteld. Dit werd geconcludeerd op basis van subjectieve en objectieve spraaktests. Zo moesten patiënten op vier verschillende onderzoeksmomenten prenten benoemen. Iedere sessie werd gefilmd en na afloop fonetisch geïnventariseerd door twee  onafhankelijke onderzoekers. Daarnaast werden de spraakstalen aan de hand van het softwareprogramma PRAAT volledig geanalyseerd. Op die manier konden ook objectieve metingen de resultaten bepalen. Het feit dat de meeste klanken door de bovenkaak en -tanden gevormd worden, verklaart de beperkte invloed van de ‘All-on-Four’ techniek die toegepast wordt op de onderkaak. 

Een positieve evolutie kon worden teruggevonden in het kader van algemene tevredenheid. Dat kan verklaard worden door de voldoening die patiënten ervaren met hun vaste prothese op implantaten. De prothese wordt als een stuk van zichzelf ervaren en patiënten winnen hierdoor aan zelfvertrouwen. Daarnaast zijn ze minder geremd bij het praten omdat ze geen risico meer lopen om hun prothese te verliezen. Bovendien vinden velen het esthetischer dan een uitneembare gebitsprothese en kunnen ze, in tegenstelling tot vroeger, makkelijker en gevarieerder eten. Dergelijke procedure is echter niet voor iedereen weggelegd.  Niet enkel door de verschillende voorwaarden waaraan voldaan moet worden, maar ook door het prijskaartje dat eraan vast hangt. De kosten van een implantatie in de onderkaak kunnen al snel oplopen tot 10 000 euro.

Beperkingen

Hoewel de resultaten uit het onderzoek veelbelovend zijn, moet er  voorzichtig mee worden omgesprongen. Grotendeels omdat de proefgroep te klein was om conclusies te veralgemenen naar de totale populatie. De bevindingen binnen het onderzoek zijn correct, maar werden mogelijk beïnvloed door dit kleine aantal. Dit is dan ook de grootste beperking van het onderzoek. “Streven naar grotere groepen zou ervoor zorgen dat er een hogere mate van betrouwbaarheid behaald wordt en dat conclusies veralgemeend kunnen worden”, aldus De Roo. Deze bevindingen kunnen alvast een aanzet geven tot verder onderzoek naar de gevolgen van implantatie in de onderkaak op het vlak van spraak- en mondgewoonten.
