Locomotor asymmetry and motor switching in Parkinson’s disease during tied- and split-belt treadmill walking

Personen met de ziekte van Parkinson kampen met moeilijkheden tijdens het stappen, zeker bij het bewaren van symmetrie en evenwicht. Ook zien we dat een grote groep Parkinson patiënten typerende blokkages vertoont waardoor zij tijdelijk niet in staat zijn te wandelen (‘freezing of gait’), wat erg invaliderend is. In het dagelijkse leven is het zeer vaak nodig dat we op niet-symmetrische manier voortbewegen. Een goed voorbeeld hiervan is het nemen van een bocht. Parkinson patiënten ondervinden extra moeilijkheden bij dergelijke taken. Om meer inzicht te verwerven in deze problematiek hebben we Parkinson patiënten en gezonde controle-proefpersonen laten wandelen op een loopband waarvan de snelheid voor elk been apart aangepast kan worden.

Uit dit onderzoek is gebleken dat Parkinson patiënten minder goed in staat zijn om het verstorende effect van asymmetrie te minimaliseren. In het bijzonder zagen we dat de patiënten met ‘freezing of gait’ minder symmetrisch wandelden en meer last hadden met de asymmetrische verstoringen. Wat de proefpersonen met de ziekte van Parkinson echter wel nog konden, is het volledig omkeren van de opgelegde asymmetrie. Het vermogen om de gang aldus aan te passen moet verder onderzocht worden als een mogelijke strategie voor de gangrevalidatie van deze patiënten.
